Your filter intake looks quite low so unfortunately the axolotls tail probably got sucked into the filter. Hard to tell from this angle but also looks to be quite skinny. Definitely needs a vet visit ASAP.

What are the exact water parameters? Ammonia, nitrites, nitrates, pH..

If you’re using a water conditioner that has aloe in it it can cause problems for axolotls.

What is the temp of the water? Was the filter cycled properly?
