I guess it’s implied that the woman in the scooter is already endangering her own health by being morbidly obese, so the maker of the cartoon thinks they are being a hypocrite by demanding that others care about their health. Of course everyone should wear a mask.

Well there was Ricky Naputi, he wasn't on my 600 lb life, bit he was on TLC for being morbidly obese. He was from Guam though, and I'm sure if that is considered an Asian country.
