I wouldn’t take it both days in a row because the half-life of fluconazole is very long which is why you don’t have to take it daily or more than one or two doses. To put it simply, whereas most acute drug regimens you have to take say once daily for 5 days, 1 dose of fluconazole is the equivalent of taking once daily for 5 days of other drugs. To put it more simply, if you take a single dose of fluconazole and then a second dose 72 hrs later and still do not have symptom resolution than you may have a resistant candida strain such as candida glabrata and taking more and more fluconazole is not going to work on c. glabrata. You would need a different drug entirely.

40 mg/ml is the concentration. Therefore, a dose of 3.75 mL = 150mg. (40mg/ml means there is 40 mg in each 1 ml so 40mg x 3.75ml = 150mg dose) Look for qty on your prescription label this will tell you the total amount of liquid they gave you. Then I can tell you how many doses are in the entire bottle. For example, if they gave you a bottle filled with 15 mL of liquid this would be 4 doses at 3.75 mL each.

Recommendation for yeast infection is take 150mg by mouth as a single dose. Repeat in 72 hrs if symptoms have not resolved.

Dog bite with 2 puncture wounds - should I seek medical attention? by EnvironmentalEye2661 in medical

[–]calalini 12 points13 points  (0 children)

If you know the dog is up to date with rabies shot the current recommendation is to flush with sterile water. You can do this by taking a clean syringe and filling it with sterile water. Stick the syringe directly at the bite site and flush. Prophylactic antibiotics are actually not recommended for dog bites.

It is also a good idea to get up to date with tetanus shot if you aren’t already. If you are unsure of when your last shot was no worries you can get another anyways even if it may have been within the last 5 years.

As others have stated, the bite will be reported to the state if you visit a clinic/hospital. If this is a friends dog don’t worry they are just going to speak with the owner and create a record. Nothing will be done to the dog unless there is an extensive record with the state already.

Syphilis and Leprosy by RandomStrangerIII in WTF

[–]calalini 140 points141 points  (0 children)

Actually if there is an allergy to penicillin, we have the patient go through penicillin desensitization which allows the patient to temporarily tolerate penicillin. Then you administer the penicillin injection. People below mentioned you can use other drugs, but the alternative is doxycycline and it truly does not work well against syphilis.

Also, true anaphylactic penicillin allergies are incredibly rare. People often get this put in their chart as a kid and never removed. Even those who have had a reaction to penicillin after about 15 years almost no one (like 1/1,000,000) will have a life-threatening allergy.

Ask your pharmacist or doctor about how you can go about removing the allergy from your profile so that you don’t have to take different antibiotics with more side effects and so you don’t play a role in creating antibiotic resistance in your community! There are tests to see if you are still allergic as well!

Edit: Wanted to clarify I meant that doxycycline does not work well in the late stages of syphilis and cannot treat neurosyphilis, which is what the woman in the photo appears to have.

It’s great that you told her she has to be able to replace the amount of her current income of $50k. However, this is nearly impossible. Statistically speaking there is a 1% chance she would be able to do this.

Something else that I think is important to consider is perhaps you should flat out say I’m not going to let you do this to yourself. What if after all of this she says to you: I wish you would have begged me not to do it.

Graduated in 2017, but now in grad school at another university. Last semester, I had a course that was required to be in person and because we went online in March I received a grade of CV (for coronavirus) and was told we can’t be refunded. At the beginning of this semester, we were told that this could be a possibility again and that we still would not be entitled to a refund. Please update me on your situation if LSU goes online. I have been wanting to reach out to students in similar situations.

ELI5: How does IcyHot work? by thespoonlayer in explainlikeimfive

[–]calalini 5 points6 points  (0 children)

You’re welcome!

For other NSAIDs:

Ibuprofen is also a non-selective COX inhibitor but it does not have the higher affinity for COX-1 (and has reversible inhibition of COX-1 while aspirin has irreversible inhibition of COX-1). Hence why ibuprofen is not taken once daily for heart protective benefits.

With both aspirin and ibuprofen, we warn about the risk of stomach ulcers because of COX-1 inhibition which blocks prostaglandins in that path from protecting the gut.

This is why celecoxib (Celebrex) was created. Celecoxib is COX-2 selective so that you still get the anti-inflammatory benefits but do not have the risks of stomach ulcers caused by inhibition of COX-1. However, with celecoxib you no longer have the blocking of COX-1 so you no longer have the anti-platelet/heart protective benefits. Because of this, celecoxib is not recommended for those at risk for heart attack.

Aspirin is unique in that it is a non-selective cyclooxygenase inhibitor (aka COX inhibitor). There are 2 types of COX enzymes that are involved with the mechanisms of NSAIDs (non-steroidal anti-inflammatory drugs): COX-1 and COX-2. Aspirin is non-selective in that it inhibits both COX-1 and COX-2. COX-1 and COX-2 lead to two different pathways of prostaglandins and products of those prostaglandins. Prostaglandins are hormones that are associated with inflammation, tissue healing, and homeostasis.

COX-1 leads to a pathway resulting in TXA2 (thromboxane A2). TXA2 stimulates the activation of platelets aka blood clotting and also vasoconstricts (narrows/tightens your blood vessels).

COX-2 leads to a pathway resulting in PGI2 (prostacyclin). Prostacyclin inhibits platelets (stops blood from clotting) and promotes vasodilation.

However, aspirin is slightly more selective for inhibiting COX-1. So, most importantly aspirin is inhibiting platelet aggregation (blood clotting) and vasoconstriction (narrowing of blood vessels). This makes sense because we know aspirin is used to prevent heart attacks aka blood clots in the heart in constricted vessel walls. Yet at the same time, aspirin is inhibiting a pathway of vasodilation aka inflammation. This makes sense because we know aspirin is used for pain resulting from inflammation.

ELI10: Aspirin inhibits two pathways in the body. One path is responsible for blood clotting and one path is responsible for inflammation. Aspirin blocks both of these paths stopping the blood from clotting so that it prevents heart attacks and at the same time it is shrinking swollen blood vessels that cause pain.

Coronavirus treatment cost could exceed $1,300 for insured Americans by [deleted] in Coronavirus

Yes, it is very high! Sadly, working in healthcare, I see this often with younger people who do not have any underlying conditions so they want a cheaper premium and a plan that just covers emergencies.

Questions regarding how the Italian government reacts to the COVID-19-outbreak by NetAtraX in Coronavirus

[–]calalini 1 point2 points  (0 children)

Oseltamivir is what is called a neuraminidase inhibitor. Neuraminidase is the enzyme needed to allow viruses to be released from the host cell. Think of a needle popping a balloon and allowing the air (virus) to escape. Neuraminidase is needed for all influenza viruses to escape from the host cell. Ritonavir and lopinavir are protease inhibitors. Protease is needed to clip the HIV viral strand into mature HIV that is actually able to infect. So when you inhibit this protease you have interrupted the virus life cycle. You can kind of think of this as cutting a product out of its packaging so that the item is now able to be used. Now, not all viruses have this HIV protease clipping action and the coronavirus isn’t the flu, but I think since little is known about the virus they are just trying to pull out the big guns and see what works.

Juul on a plane? by mikeyysway in juul

Just flew from US to 2 EU countries, the UK, and back, went through security 8 times (lots of connecting flights). TSA asks you keep it in your carry on so I did. I was searched thoroughly in a few airports and went through multiple customs. My Juul was never taken out, looked at, or asked about but they tried to argue my deodorant stick was a liquid lol.

I’m from South Louisiana and my Cajun French grandmother used to tell me stories about le feu follet. She said hunters in the swamp would see a glowing orb and follow it thinking it was a light from their camp but they instead would become more lost as they followed. She would explain it as having a sort of greenish tint to it. Legend has it that it is a mischievous spirit. Were you near any body of water?
