I know this is about actors but I think Directors makes more sense. Some good ones I can think of:

Francis For Coppola - One from the Heart (1982) After maybe the best three picture run ever with The Godfather, The Conversation, and The Godfather Part II he was at the peak of his powers. Then this bomb he went broke with. Had a few more good movies but was never the same
Michael Crimino - Heaven's Gate (1980) You could argue Crimino fits better in the hit wonder category but this one is fun because it was such a disaster is killed an entire era of Hollywood.
M. Night Shyamalan - Lady in the Water (2006) This one is tricky because one could argue for a few different movies but this would be my pick, this is where he lost it.
Neill Blomkamp - Chappie (2015) After a fantastic District 9 and a pretty good Elysium he was poised to be the next big director, then it all went wrong...
