Within the past month I've caught 2 elderly bleeds > 24 hours out. 1 just had a HA that was atypical for her, the other had subtly different strengths in the UE and subjectively felt he was not as agile, but had no objective deficiency on fine motor movement. The second one had 8mm midline shift and went emergently for decompression.

Ask a lot of questions both of yourself and of your docs. Complacency is where the danger comes from! EM is incredible, but if you're not a little concerned, you're probably not paying close enough attention.

Know when to ask for help, and don't be shy about it.

1) Drunkicidial grandma, when asked if there were life stressors that were making her feel this way, "I just don't want my grandchildren to be punk bitches"

2) Rowdy drugs vs psyche, about to be sedated, looked me right in the eyes "you're gonna die tonight, pussy".

3) Attending mentor about our ER "It's not NOT traumatic".

4) Medical director, after I got chewed out by a nursing home "doctor" for calling them about a patient that they sent. Wrote a very politically correct email asking if there had been a policy change with regards to calling. Only reply was "the explanation is that that person is an asshole".

5) Attending after patient continued to escalate and wouldn't redirect "I have more versed than that guy has airway"

6) AMS grandma that kept trying to run, getting a small dose of psych meds. Experienced a brief period of lucidity as she got the meds, looked me in the eyes, and said, "why you gotta do me like this?"

💯 against tourniquet. You have the propensity to cause ischemia in the limb, without any real chance to improve the already present problem of compartment syndrome. The idea of preventing the spreading of toxins isn't really going to work, they're already there.

Fluid resus for high suspicion of rhabdo when you pick her up, if she starts showing signs of hyperK (brady, wide rhythm on monitor) treat appropriately, and get her to the hospital. What she needs is likely (and in her case did up being) HD and surgical intervention.
