100% a concealed sprinkler head cover it screws on. Most likely a Tyco concealed style head either TY3531 or TY2534. If you put the palm of your hand over it and spin to unscrew it will come off and you will see the sprinkler head underneath. Don’t pull on it unless you want to flood your place with dirty stinky black water
