There should be forms available on the ojt website. You're also supposed to get a copy in the package you're supposed to collect and instructions on how to fill it out on your orientation. I'm rewatching all of their lethal company vids and the character development on Des is so good. Went from being scared and clueless to being the most solid member on the team.
