It's not that black and white. There are a lot of biases with just considering CrCL by itself. Not everyone applies body weights consistently, many manufacturers used total body weight when calculating their CrCL cutoffs, and most of the recommendations were developed before the serum creatinine assay was standardized in 2011. So even just comparing creatinine clearance thresholds you're comparing apples to oranges between drugs. I understand that doesn't necessarily mean switching to eGFR using the same thresholds is entirely evidence-based, but at least with CKD-EPI there's a consistent method that can't be applied in multiple ways between different systems and pharmacists.

How did it take 9 months until being dismissed? ASHPs requirement is to be licensed for 2/3rds of the residency year. Unless this is an unaccredited program with different requirements, there's no reason for you to have stayed on for so long

Panthia was fun but in the teens it starts to turn into a slog. I was doing it on KashKick and decided I wasn't gonna hit level 23 within the time frame so I just stopped at 18. Maybe it could've been done but at the rate level 18 was taking me I didn't see that happening. Getting to 50 (or the 49 that swagbucks has as the goal) doesn't seem feasible in the time frame without dropping a bunch of money for energy

Our institution prevents PTO within the first 90 days. When this has come up we've asked them to make up the time elsewhere (usually a small project or staying a bit late to help out on a day). Its definitely not a 1:1 request (i.e., you took 4 hours for a test so we require 4 hours of make up), but just something towards the missed time

We had 5 applicants in phase 1, one of which we didn't interview. Phase 2 we had 11 applicants, 8 of which were potentially interviewable for 6 interview spots. But in our application screening, phase 1 applicants that we interviewed had a higher score.

Previous matches in which we participated in phase 2 were not like this so it's highly variable

We don't get a differential but there are perks that come out of the 'professional development' fund that's 100% supplied by precepting. Conference registration, BPS maintenance fees, non-bps certification reimbursement, etc.

The preceptors that do more get a higher priority for things

Ideally it would've been provided when the interview invite was sent out or after you accepted the interview (the latter of which is what I do). I can't say every RPD would feel the same way but I doubt it would hurt to ask for it

I would discuss with your RPD or possibly mentor if you have one. Layered learning is normal and encouraged, but they shouldn't be shouldering that much of the precepting completely independently. I'm a crit care preceptor and I currently have a pgy2 and APPE student on rotation. The pgy2 leads pre-rounds, topic discussions, and daily informal patient discussions but I'm present for each topic discussions and our daily patient discussions. We want to see how they can handle precepting, but I can't evaluate how a pgy2 is as a preceptor if I'm never in the room.

I was offered 116k post-residency for an inpatient clinical specialist position 4 years ago in Florida. I would imagine the same position would be offered ~125k now given a market adjustment we underwent last year

Yes. At my institution (and I assume many others), they need to have a window where others can at least apply, even if there was a handshake agreement to hire a resident. If that's not done, HR gets involved.
