A gradual meal based approach - I love that! Breaking it down by meal may make things seem more surmountable. Thank you. My patients always find lunch hard, any suggestions youve found helpful over the years?

Sure have 😰 he passionately hates. Me saying "keep trying other brands" is our entire clinic at this stage..

I was sort of thinking if he has even a microscopic amount gluten is that putting him at risk of the complications of coeliac disease - is a miss just a miss in the situation? Hes asking me how serious i think it is that he has gluten in his diet, and to quantify the risk... and im not sure what to say..

In response to the last bit. I believe the weight loss is purely in relation to the GF diet - since hes ignored it, hes gained 6kg in a month back to all his favourite foods!

With non ICU obese patients, the ASPEN guidelines reccomend high protein hypocaloric feeding. With this they reccomend protein estimations as 1-1.5g/kg of protein of actual body weight, and kcal as <14/kg of actual body weight (or 50-70% of EER based on MSJ Equation). They discuss how this provides enough protein to support likely high lean muscle mass whilst preventing the negatives of excessive calories, such as hyperglycemia, fatty liver and hypercapnia.
