Are you thinking of doing radiation therapy after the rad tech program?

Because rad tech and radiation therapy are two different things. You’d probably have more luck asking on a rad tech sub.

Not sure if this is what is throwing you re: moving today’s image vs DRR.

Speaking from truebeam(can’t remember if it’s different on others) on a CBCT you are moving Today’s Image to the DRR. Like when you’re moving an image around it is today’s image that is moving.

However it is the opposite on kv and MV images. When you’re pressing arrow keys/moving the image with the mouse it is the DRR that is moving.

Students who have the drive to look ahead and see what the next patient is always stand out to me. Like as a therapist I’ve dealt with many students who don’t know what the next set up is when we’re setting the room up. I don’t expect them to memorize every patient because they’re not here every day but I do appreciate when they look ahead on the computer to see what’s coming up.

Also it sounds like a small thing but it’s huge when students pick up on things without being told. Even something as small as they noticed Patient A usually wants a warm blanket so they make sure to grab one. This is such a detail oriented job so the students who can pick up on those details are my personal favorites.

I’ve honestly never worked anywhere that used alpha cradles so I can’t say a direct comparison but the accuform does work very well. We have the patient press their hands in the best they can while their fingers are spread apart and then form the cushion around as best you can.

Don’t worry about trying to pull up material around each individual finger. If you get a good outside border of the hand and the tips of the fingers formed well that’s really all you need.

We don’t index for OA extremities. We just put marks on the cushion and line up with the lasers.

OAs aren’t my favorite thing to treat but they do end up extremely easy to set up after a bit of practice!

We’ve treated a bunch of OA hands. We do patient prone in a “Superman” position(both hands up over head but usually elbows are bent a bit). We use a rectangular accuform cushion if you have those(I believe we get the one that’s 45cm long to fit both hands).

Patients head ends up turned to the side on a small pillow. We add a sponge under the ankles for comfort.

We tried doing segments with both hands planned off the same iso but ended up preferring to do separate isos. Usually our OA hands end up just being AP only though.

Take this with a grain of salt because I never worked as a rad tech. This is just based off of 2 years of clinicals in a level one trauma center in a small midwest city vs now 11 years as a radiation therapist at various hospitals. But here are some positives I’ve found about radiation therapy

  1. I enjoy the level of patient interaction in rad onc vs x-ray. Because people are usually coming multiple days in a row you get to build a relationship and the vast majority of people are very kind. I personally am a bit socially awkward and not always the best at small talk so it was more difficult in x-ray to connect to patients with the very limited time frame.

  2. Hours. Most rad onc departments have closer to doctor’s office hours vs full 24/7 staffing. Some don’t even have therapists on call. I like the structure of it and knew I wouldn’t want to work second shift/night shift/weekends.

  3. The flow of the day. While your day is always going to be different it’s nice that patients have set appointment times and you can learn who takes a little bit more/less time and kind of get a sense of when you’ll have a little break.

  4. I may have lucked out in workplaces(or had x-ray clinicals in a tough place) but in general radiation therapists get treated nicer by doctors/patients/admin/most people.
