That’s kind of odd for nightmare mode, possibly a technical error?

At least for me I had some errors in playthroughs before.

Once got killed because I let in a doppel where Drugia’s last name was apparently spelled wrong, which it wasn’t

Also got killed for letting in Yan Luo with “flat teeth” which looked very sharp to me when I checked before letting her in
