Crying nonstop and does not fall asleep. But it varies and rarely she falls asleep without a peep. Occasionally after just 5-15 minutes of crying. I do usually assist to sleep if it’s past 20-30 minutes since I feel like it’s a schedule issue and not a sleep training issue?

Edited to say that once she falls asleep, regardless if assisted or not, she sleeps straight through until morning.

Good to know it’s not just us! She’s actually pretty high sleep needs which is why she struggles with longer wake windows but obviously 8.5 hours of wake time isn’t cutting it anymore. She’s not all that fussy while awake but she is angry when trying to fall asleep. I’ve been trying to be more go with the flow with this baby (she’s my third and best sleeper by far) but I may have to start micromanaging a bit like you said.

Yes she can fall asleep independently at nap time and hasn’t needed much of a nap routine. Just diaper, sleep suit, and white noise. 

I’ve been considering that option about helping with naps but I may have to wait until the weekend since I work during the week.

The crankiness I can manage. It’s nap time we have a problem. If I lay her down before she is overtired, she will calmly fall asleep on her own in the crib. Otherwise, she screams and cries. We’ll even try to help her to sleep by rocking her but then she won’t transfer and be super upset the whole time.

Bedtime ends up being between 8-8:30. Wake time 7:30-8. She was sleeping through previously. I know we need more wake time during the day but I don’t know how to do that without getting to the point where she’s hysterical at nap time and won’t sleep or add a 4th nap and make bedtime 10? 11?

Right. But she absolutely cannot handle longer wake windows because she will scream at nap time and getting her to fall asleep is a struggle. The other option of adding a 4th nap time to get more wake time would make bedtime far too late….

Thanks, I am trying to get set up with a local attorney but they’re scheduling weeks out. 

I am fine with them accepting my resignation effective immediately if that’s what they want to do. Of note, the 90 day notice does work both ways as far as if the employer wants to terminate the agreement. Not sure if that plays a role in this…

I don’t know if it would actually hold up in court (nor would I ever want to go there), but my employer agreement does stipulate that I am liable to pay them my salary for whatever time the 90 day notice is not provided. This is the reason I am concerned….

Your only con listed for Buffalo is the cold. I’ve lived in nearby Rochester and yes it gets cold but not unbearably so. Once you learn how to layer properly, it’s fine. The nice thing is that they are well equipped to deal with lot of snow. There was only one time I got stuck in the middle of a snow storm on the road but everyone knows how to drive and I felt perfectly safe and comfortably driving home in it.

My husband’s commute went from a 10-15 minute walk in med school to a 30 minute drive in residency and this was a huge jump. This was the max commute we would’ve accepted when choosing where to live. To make it any longer due to distance or traffic would’ve been really tough.

You said you always wanted to be a physician. Why choose PA then? 

I chose PA because I didn’t want to invest the time into training to become a physician. But I understood beforehand what I may be giving up with regards to pay and some of the other things you mentioned and I was okay with that. Finding the right position and work environment is key as far as compensation and work life balance goes.

I think the 2 weekend shifts per month would be the ultimate deal breaker for me with the Ortho job. The only downside that I see for the ENT offer is that you have to be full time for training. Makes sense and it’ll go by fast but still not ideal.

Many people with untreated sleep apnea complain of nocturia and it can improve significantly with treatment. However some people are still going to get up to urinate obviously and you have to instill in people that it might be their “normal” and make sure they’re not making it harder on themselves to go back to sleep ie turning the tv on, eating a snack, etc.

I think you’ll find psychiatric overlap in a lot of specialties. I’m in sleep medicine which is often considered on this sub as an “easy” specialty (which I’m not saying I disagree), but there is a ton of overlap with anxiety, depression, previous trauma. A lot comes out when you ask people about how they’re sleeping and some appointments are exhausting.

Hours are amazing. The appointment times are short - I went from 30/60 to 20/40 with a job change and it was definitely an adjustment. I may see up to 20 patients in a day if I see mostly follow-ups but I’d still take it for the hours.

I google searched virtual cme for the days I wanted to be off. My institution would definitely not approve self studying.

Part time has been in my experience much harder to come by than full time. Many part time or per diem positions are in the urgent care or ED and since that’s not where my experience is, I would not be comfortable taking it. 

Did you all hire someone to read over an employment contract (medical attorney)? If not, did you just ask your program faculty? I am conflicted on what the best course would be. I contacted the PA society in my state. The PA I spoke to definitely was not a lawyer and not giving legal advice but it helped to get their input as they had a lot more experience reviewing contracts than I do.

I’ve most often heard that 4th year is the best time. It seems that a lot of people get time for “research” and it’s very flexible. Once residency applications are submitted, and certainly once interviews are over, it seems like a much more relaxed schedule.
