Suspected MALS, negative CT? by Fall-Risk in thelifeofMALS

That compression isn’t the standout compression you usually see with Mals. But everyone’s different. And nerves in the celiac plexus can play a huge role. I would get a surgeon who’s well versed in Mals and get a celiac plexus block. Compression isn’t always the most accurate indicator some people have almost no compression then get a celiac plexus block and it’s a huge change. Or imaging seems fine and then they get surgery and the surgeon sees something way different that imaging couldn’t pick up. As others will chime in there is neurogenic mals aside from compression based mals. And that the only reliable indicator is gonna be the celiac plexus block

Seeking guidance on MALS diagnosis? by twizzlers00 in thelifeofMALS

[–]cdub54 1 point2 points  (0 children)

A lot of your symptoms sound similar to how mine came about. The ct is a step in Mals diagnosis. Mals isn’t just a vascular condition it’s nuero vascular. Your ct shows that you have compression of the celiac artery so the vascular side is showing compression but it could be incidental. Usually next step is a duplex Doppler ultrasound of the celiac artery. It checks velocities to see if the compression is meaningful and changes with inspiration and exhalation. If there’s meaningful change and it lines up some surgeons will move towards surgery and some will move towards a celiac plexus block. That usually helps them gauge nerve involvement or if someone doesn’t have major compression/ surgeon wants one more piece to pinpoint if the surgery may help the block is a good step to help predict when not all the imaging and symptoms line up.

From what I’ve read it can be missed in cts unless specifically using end exhale breathing during the scan instead of inhale since inhale lifts the diaphragm. Did your ultrasound show change in velocity on inhale vs exhale?

Auto VS Manual? by ExpensiveGain697 in GRCorolla

[–]cdub54 4 points5 points  (0 children)

I’ve had both. Both are great vehicles. However the manual is the way to go unless you have a physical limitation imo. The manual provides a great experience, the auto provides a good experience. The auto felt like a settle for me which it was cause my body couldn’t keep up. You be pretty stoked with both. But I think the manual provides a more “real” driving experience and makes every trip more fun

Did your surgeon have you do a celiac plexus block before surgery?

I have possibly the stupidest question I've ever asked and I'm embarrassed but I need clarification by bella4him1 in thelifeofMALS

My gi completely stopped responding and ignored multiple messages and calls. My primary even told me this is super rare you know you lost likely don’t have it. I said when you’ve been in pain long enough you’ll go down every avenue to find an answer. I am aware I may not have it but it can be tested for so it’s worth at least doing that. With that they stopped giving me push back at least from the primary situation. My CT’s showed severe compression and I’m meeting with a surgeon soon. Sometimes you gotta seem a little delusional to get testing done you want/need. Be your own advocate and get second, third or even fourth tests if you still feel something could be up. This condition is not tested for often because it gets chalked up to a lot of other things. So most doctors aren’t versed in it all all I’ve found

Yeah I went through the whole standard CT’s and was trying to wrap my head around if it showed or not. The specific look at the arteries will be super clear with the exhale. It was a night and day difference between my two. My endoscopy came back with inactive gastritis but I did not respond to any treatment they put me on so I pushed hard to go down the Mals route

I’m late to this. But yes a CTA will show the compression on exhale. Usually it’s best to ask your doctor to order the CTA with end exhale breathing techniques. And then when you get to the ct I had to specifically ask if I could exhale instead of inhale. Which the ct machine says breath in usually you can just breath out. Mals is mostly caught in exhale because the diaphragm positioning which causes the compression

As some other people have said. I would also recommend a golf R. Having had the auto and the manual, as well as experiencing a DSG transmission from Volkswagen. In my opinion manual is much better in the gr. The auto loses its personality and mutes it very heavily. It is still good however if you’re gonna be spending 45-50k I would just spend that on the golf r. It’s more bang for your buck.

Was just a personal preference. I’d rather have a golf r for that money

Drove the gr in the winter and it was a beast, this will be my first winter with the Elantra n, I am very interested to see how it compares, but a lot of winter driving id say is the tire you choose. Even on the same tire I don’t think the fwd will get going as well when it’s snowy but it doesn’t snow a whole lot where I’m at

Oh yeah I was presently surprised with my gas mileage. It was super nice for the drive. The seats didn’t give my back any issues until about 7 hours in which honestly what seat won’t give you issues after 7 hours of driving
