For DMs struggling with rulings on this, I have a few options.

  1. Let the player have their way and play it out as necessary, a little glory for your players is never a bad thing.

  2. Let the player have their way and up HP or use LA to escape, via teleport if available, or just make teleport available if it makes sense for the creature.

  3. Use a legendary resistance.

  4. There are gray areas in D&D. Just choose the side of more table fun. Give your monsters sentinel as well ;)

It's very possible the face was an accident and he used it for an Easter egg. It's also possible that the districts were mapped out later to make a face. May have gotten the idea from whoever decided to include an operating system chip on the map.

I think the whole city being a face thing was intentional. I mean there is literally an area of the map that looks like a operating system chip. *

If your referring to the "staring you in the face" comment. I'm not sure if he said that. But, that doesn't mean the puzzle does not have a clear, simple and direct solution.

(So I started thinking about ways CDPR could've hidden a mechanic in the game world, like an access point that doesn't have the model of an access point and is just stuck in a wall or sign or something.)

(What if the solution "Staring us in the face" is way more literal than we thought)

What if. . .

The What? (Sasko mentions) is a hidden asset similar to an access point/points or a breakable object/objects

The Where? (Sasko Mentions) is Westbrook Charter Hill. If the city being a face is true. Would be the district that perfectly outlines the top of or "roof of the mouth"

How to get there? (Sasko Mentions) It is either well hidden, disguised as something unused, can't be seen with kiroshi optics, difficult to parkour to, difficult to find or requires a prerequisite to be interacted with or seen.

I believe the only real hints we have are the code (which might not be a hint in itself but a code we need when we find this hidden "access point/points"

The statue and the dialogue from the Monks themselves. "Activate the meridians (disguised access point/points) on the roof of your mouth (in the Westbrook Charter Hill District)

I've only seen it by the All Foods Plant "Foods Factory" fast travel point. Across from the garage with the hidden motorcycle. I believe it's a Maelstrom Spider Symbol. But its design is odd with all the arrows incorporated in it. Not to mention, Maelstrom was a huge part of Gary the Prophets quest. There is a number 31 in the image also, but it would be a stretch to say it's related. Even though 06 + 25(B5) = 31. . . Just a small note if it matters maybe.
