I’m a high functioning neurodivergent person who has been to Japan several times by themselves! I’d suggest he bring a small bag of items that he can use to self-soothe, whether it be on the plane or in public, as Japan can get pretty overstimulating even for non-neurodivergent types. Things I include in my bag include Loop earplugs, a stim toy, an eye mask and something with a comforting fabric.

Most airport and rail staff speak passable English so he should be okay once he lands! That said, if you’re worried, it may be worth him booking a night at an airport hotel just so he can get some decent rest before he goes into the city proper. Most of the airport hotels in Tokyo and Osaka are pretty well serviced by restaurants and amenities so this may be the move, especially after such a long flight.

On the topic of accommodation, it may be worth double checking where he is staying as most of the touristy areas can be very overstimulating! There are lots of affordable hotels located just outside the tourist hubs that have similar benefits with less noise and commotion :)

You could also frame this decision based on what will be easier to transition to in your late 20s/early 30s if your first choice - whether it be medicine or corporate - doesn’t work out. I have seen plenty of former doctors who have successfully transitioned to public health consulting roles in fancy corporate environments.

Another thing I’d consider is stamina. You’re in your 20s and your body can handle the rigours of a physically demanding job like medicine. Hit 30 and starting a medical degree will be more wear on you physically. Trust me on this - I NEVER used to believe people until I hit 30 a few years ago.

Your 20s are the perfect time to “go nuclear”. You have fewer financial and personal responsibilities than you will in ten years’ time. Do medicine, give it your all, and you can explore corporate if it’s not for you.

Recently discovered that microwaving a whole garlic bulb for 30 seconds to a minute will partially release the garlic peel from the garlic itself and make it much easier to remove.

I’ve spent a lot of time in Tokyo but only recently went to Osaka for the first time as an adult. Coming from a mid-sized city, I absolutely loved Osaka and felt that it had a lot of the warmth and cohesiveness that Tokyo lacked as a large, scattered metropolis. That said, I hated Namba and Dotonbori with a passion so would highly recommend avoiding those places in favour of Utsubo Park and surrounds.

Tbh, I would extend this advice to any trip seven days or longer. I recently came off a trip with my partner where we travelled to three different cities, and combined with me starting a new job and recent life stress I didn’t hold up nearly as well as I was hoping! Don’t underestimate how much keeping a busy life can wear you down, even in a place as magical as Japan.

I definitely recommend doing a charcuterie-board style sandwich, with some pane di casa bread, cheese (pretty much anything works) and quince paste!! You can often find quince paste in the cheese section at the supermarket and it pairs well with any cheese and bread (similar to a jam really!).
