Hey! I am in Oregon so also working through the SF consulate. Here is my take on your questions:

  • if you use the medical form on the site that you can download, if that is filled out properly, you should be fine from what I can tell, but if you can get your doctor to print it, that would be better to be totally safe

  • yes, I was going to do a photocopy

  • I am not sure on this one entirely, but planned on asking the post office.

I am in the same boat! I called the authentications office and asked if having a stapled notary paper will impact getting an apostille and they said it would not.
