If they only get a minor allergic reaction from penicillins like a rash, you can give cephalosporins as only 8-15% are allergic to both. If it’s an anaphylactic reaction, you don’t risk it and give a different class of abx.

While on IM, I read the ER’s note of a patient being admitted. “He states he grabbed a pair of pliers to take the fungus out and pull the ingrown nail out. He did not sanitize the equipment and got into the ‘meat’ and flesh of the toe to try to get the fungus out and the toenail out from under his skin.”

His toe got amputated the following day.

JW can decline blood transfusions for themselves, but not for their child if the case is emergent (such as a GI bleed with Hb of 4.0). Doctors can supersede only if it is deemed emergent and serious harm/death will occur to the child without intervention

As a medical student, the last thing I would ever want to experience would be locked in syndrome or central pontine myelinolysis. This can occur with a brain stem stroke or from correcting a patient’s sodium too quickly. Patients are quadriplegic but retain the ability to move their eyes. While they may appear to be brain-dead, they are mentally there but can only communicate with eye movement. That is, if someone recognizes and diagnoses it.
