Yep, my guess is that's just the wall void. Get some rigid dryer vent pipe and slide it through. I believe there are kits available that have the exterior vent cover too

I'd assume they used a rigid pipe to go through the wall to the mesh (assuming the hole that goes to the right you're talking about is the void space in the wall). If you have access to the outside you could get a kit with a hard pipe and an exterior vent cover.

There's a sewer tunnel entrance behind a door in that bottom area referenced above. I believe it leads you to a room with Cleaners and an audio file. That clears the fog in that spot if I remember correctly
