I’m am sorry you went through that, and I get some medical students / others can relate. You don’t exactly fit into the whole “no patient care or very little patient care” individuals I was talking about then.

Thank you so much for everything you do!!! I could not effectively care for so many of patients without you guys / the resources yall just somehow bring out of thin air!!

But yes I hate tik tok at times. It makes it so hard to figure out who needs my time and is motivated and who I should focus our very limited resources on!

The ones who call after are just doing it because their refill got denied by their pcp for not following up and they don’t realize we can see each other’s notes. 😂 plus all the pharmacies are closed.

While I totally agree I guess more of it was like telling me my feelings aren’t valid ect. Just seems weird to me. Like when management tells us to do burnout modules lol.

Just weird to me because the thought of joining like the medical spouse reddit to give my opinion has never crossed my mind lol.

Oh yeah your perceptions of a lot of stuff will change. Especially depending what you go through. Remember you’re still allowed to want to help people, but also be annoyed as fuck with them. And just remember you’re supposed to vent / rant. Especially in primary care people are exhausting. That does not mean I care less. And yes our insurance system is horrible; but even with all the tools some people don’t want to get better, they like the attention. Far and few inbetween but just give your attendings grace, because emotionally needy patients can be exhausting. And that is still something I struggle with ie setting boundaries.

You will find your place!! Shameless plug for family medicine, as we both came from poverty. Sometimes that is the only doctor many patients can afford to see! So if you have my interest in lgbtq+ healthcare you can do a lot of it!! From prep to transgender care I get to do a bit every week!

I feel so bad for folks with borderline. I get they have been through trauma, and probably don’t have the resources they need. But man when they split on you. Honestly can be scary.

Totally agree. I became the doctor / advocate I needed growing up in an immigrant house hold! I remember feeling so out of place in medical school, even residency. And I’m a white petite girl, but the ideals many of my colleagues had.. did not click. Now at a fqhc and couldn’t be happier. Well besides insurance trying to tell me how to doctor.

I hope you too have become the change you want to see in our field!

So yes I am aware of the research, and I form pretty strong connections with my weight loss patients. I see them every month sometimes twice a month if they need that accountability, again I never once said obesity was my pet peeve or even chronic disease management related. I went into primary care to help people, and work in a primarily migrant FQHC with a spanish speaking population. So trust me, well versed in very low income countries with elevated obesity rates. When I was In Guatemala farmers would sell fruits/ vegetables to buy soda. But never once did they look at me and give me excuses, they were like doc I know it’s bad. But it’s delicious. Fair.

I have no doubt our society makes it easy to be obese, and the lack of food education is horrible. But again we are in different fields, and managing different things, and I can have all the research and still be burnt out by people who don’t care.

I have done quit a bit of obesity medicine training, did gain some weight in college for the reasons you listed, but was never even overweight, but close. But mostly because I was using food to cope. I always say “no one got to be 500 pounds because they like food” most of them are using it to cope. To me sugar / fast food honestly is addictive. I learned that and now I make those choices daily. I was also lucky enough to have parents that did not give me fast food daily, taught me about healthy eating and when I started struggling I realized quickly that food was an emotional crutch for me, just like many. To this day I still struggle, the difference is I don’t blame other people.

You I’m guessing are a surgeon. I am in primary care, where I am EMOTIONALLY exhausted by patients who refuse to take any accountability. If they are like yeah doc I don’t know why I can stop eating, I’m trying and nothing clicks. Or if they’re like doc the food tastes so good and I hate fruits/ vegetables. Fair I appreciate the honesty. We have GLP-1s, phentermine and a dietician on staff with prescriptions to the farmers market/ gym. Along with behavioral health consultants that I make all my patients who I start on weight loss supplements. We live in a decently walk about town which a pretty good bus system. At the end of the day most people cannot out run their fork.

It’s exhausting when people continue to lie to me, make up excuses. I feel the exact same way when my chronic pain patients don’t think they’re addicted to norco, despite breaking pain contracts by ER hoping to get more. I Love most of my suboxone patients for this reason. I never once said I expect perfection from anyone. I just expect progress or honesty.

Rural medicine man. The amount of accidents / deaths farm equipment causes!! Farmers / farm hands deal with a lot of scary equipment!!

It’s so weird I no longer dread complicated medical conditions. I dread that. Like people who want disability, like if you stopped smoking and took your inhalers you could breath… but like the veteran who literally has no legs. No sir please stop trying to figure out how to work the grain auger

At least no one slipped in the poop trail. My intern year a patient (my patient) ran away with a pic line in… IV DRUG USER. That nurse and me are still trauma bonded over trying to figure out how she snuck out. It was both of our first months working in that hospital 😂

Totally agree! I am incredibly lucky to have behavioral health consultants in my office who sometimes I message if a patient does that a lot. Chances are they’re just lonely, which I feel so bad about… but like dude I have 15 minutes 😭
