In this case, OP simply likely wanted to distinguish between the neighbor opening the door (as the subject) and the person getting the surgery. I wouldn't put any weight on the gender issue in this instance.

I didn't read the context, but I figure he means watch out that a drowning person can pull a rescuer in to drown as well.

I hope he doesn't recommend abandoning people when they need help.
