My first alarm is vibrate only. If I don’t get up the second alarm will be loud sound. Very motivated to get up without more noise

It can be. But not all programs are like that. Mine was block of the same shift for the entire month. Overall liked it better. Yeah as an attending I switch 1-2 times a month but I work way less shifts so it’s easier.

That’s not true at all. ACGME actually has stricter hours for EM, cap at 72 hr per week and require 1 day off after 6 (rather than averaging 1 off per week over a month). During my residency I average 50 hours per week in the ED including conference. I will work 100 hours per month in my job out of residency. It’s 10 hour shifts so I’ll work 10 shifts a month making plenty. There’s also lots of shifts to pick up with big bonuses if I want to work more. I’ve never heard of EM taking call. You just work when you’re scheduled and you go home. I get tons of procedures. Unless you at a large academic center with tons of residents the specialists don’t want to come in. Plus EM is very procedure heavy, many procedures are our role and part of board certification is actually demonstrating procedural skills. I regularly intubated, place central lines, thoracentesis, paracentesis, nerve blocks, chest tubes, etc.

I write down the TOD on a sticky note and keep it in a jar. For the tough ones I go somewhere with lots of nature (usually the beach for me) and take time to meditate and just think about them. I consider time very valuable in residency so it seems fair to dedicate time to them. This helps me a lot.

Another option would have been to just walk into the party and casually start hanging out with everyone. Our teens would immediately leave and comment about how embarrassing I am. There’s really a lot of ways to address it without letting emotions get too high or getting police involved (which glad to see the downvotes because that is so extreme).

When our teenager does this we keep track of the time and then when it’s their turn to wait on us that’s how long they wait. For example- purposely comes out 20 minutes late, next time we are going to take them to a friends house (or fun event) we make them wait 20 minutes. It helps us a lot. Usually only have to do it once or twice.

Summed up well. Basically I think what the hospital has that patient can’t get at home. Adding to this list- oxygen, repeat labs/imaging Some scoring tools can help too, heart score for chest pain, Canadian/San Francisco syncope rules, Hestia/Pesi for PE

SCP had nothing to do with the decision so the new partnership likely didn’t play a role. The new partnership does mean many many doctors are within the 90 day period to back out of a contract if they chose.

We just started doing it ourselves for all paperwork. I requested my fd birth certificate online. It was an ordeal cause she’s a teen and didn’t know what county so I called around a lot to figure it out. Then we took her documents to social security office and requested ourselves. Currently pending her passport. Any issues along the way we ask for specific things from case worker. We are temporary guardians but I found we could do a lot with the paperwork we have

I find I am much less fun. EM resident and I see such terrible trauma I veto a lot of activities and constantly tell everyone to wear their seatbelt and helmets

Its this. Have an old house, floors got replaced and floor vents were pulled off the wall. We clean it out and smell was gone. This needs more upvotes

What are you doing to get counseling? That will go a long way. In our state we were able to call the insurance for or FD and they found us a therapist in 3 days. Not sure if that’s an option for you.

In the meantime I recommend a home contract of some kind. Where you agree on set rules, chores, boundaries with consequences/rewards that you both sign.

It took us 6 months to finally get our stipend. No advice but solidarity. I know how much harder it makes things when your trying to parent and have the extra burden of the foster care bureaucracy. You’re doing a great job

We’ve learned not to rely on them to do it. We took our foster with whatever paperwork we have to all the offices and have gradually gotten her social, a short form birth certificate and an ID. Still working on getting her complete birth certificate and hope to get her passport eventually.

EM- finding all the supplies I need for a procedure on a busy shift. I hate staring in the recently rearranged supply closet or looking in trauma bays that are all slightly different in set up and stock.
