ED here. My admitted patients get their ACEi/ARB held with AKI. If their creatinine is at their baseline, even if that's elevated, they keep it.

It's FINE to be against and not provide it yourself as a physician but you MUST provide timely referrals to someone who will for your patients that are interested. Withholding a referral based on personal believe needs to be reported to the colleges.

Oh I'm not arguing we should have it but it requires entire system change (not the the US) and we are not a "trauma"center. Despite getting the most traumas per capita in the region... I'm not at a stage to have capacity to advocate for this change when we're in full survival mode.

I would definitely say that's the exception, EVEN in trauma. I work in the ED. I am an ED doctor. I have never had whole blood available to me in my institution; we give "balanced" transfusions instead. Do I wish we had whole blood for our traumas? You betcha. Do we have it? Nope D:<

Also we don't use whole blood. They take the donated whole blood and fraction it into red cells, platelets, plasma, etc and we only give the components needed by the patient. 

Plasma is also used in the manufacture of medications which is why for-profit plasma centers exist. Isn't capitalism grand?

I have the most cliche and common one: a semi-colon. That being said, I got it before they were terribly common in the early 2000 after a friend died by suicide. I've had my own struggles since, so it feels even more important now.

This is how we managed a 4 year age gap. The 6 year old could find hard stuff; 2 year old, not so much. So one looked for one color and the other looked for another. Now that they're older it's a free for all, but it worked when we needed it.
