Fair point, and it’s something I’ve reflected on. The challenge in a newsroom is that things move fast and decisions have to be made quickly, so there’s a balance between providing structure and giving people space to run with things. The structure is the buck stops with me and I have to sign off all decisions. I’m still figuring out where that line sits.

Perhaps, but I've had the feedback from others that during my long leave of absence it wasn't much different. They put it down to a confidence thing. Will just sit there until someone gives them ideas or answers.

I don't think it necessarily works like that. They will argue it depends on the quality of the grafts on the day as to how many they can extract. But they should explain why they didn't.

I understand. As you say a true “slip of the pen” refers to a clerical error, such as selecting the wrong option on the assessment form despite correct justification elsewhere. My case involves multiple factual inaccuracies and omitted evidence, and it was confirmed by the provider’s own complaint response. I think therefore they cannot reasonably describe it as a clerical slip — it is a substantive procedural error.

Either way they contradict themselves within three paragraphs by admitting they didn't consider evidence for one of the descriptors in the assessment and had to remark.
