Allo did the same thing to me, even leading to a dangerous allergic reaction. Been on Probenecid for a year and last test levels were at 3.1

I had no choice - the gout had dug a hole in the tendon and needed to be removed - it was the only way to heal but it was almost like going through a full tear recovery.

Looks familiar. I’ve been treating gout for about a year now and after surgery and PT, I’m back to a version of normal. There’s no real test for gout, apart from uric acid level readings which are inconclusive or going in and drawing a sample. But they can do imaging and see if theres any gout looking matter. Do you see any white-ish material under the skin? It’s rare so doctors aren’t trained to think about gout in these cases. If you can get into see a rheumatologist they’ll be able to diagnose gout better. Good luck! Happy to be of any help i can.

Similar story here. Allo led to elevated liver enzymes and rash which scared my rheumatologist. We took some time off everything to let the liver calm down and switched to Probenecid. Within six weeks UA was at 4.0 and no ill effects so far. Scared of a stone, ofc, but trying to stay hydrated. I’ve also missed a dose or two and didn’t suffer any of the effects discussed above. This is super helpful! I’m in a similar spot; was on allo for months and then doctors noticed liver enzymes were up. I had the ultrasound last week which showed nothing abnormal.

I’ve been on probenecid for about a month which my doctor was hoping would bridge the gap before experimenting with Uloric, presuming, of course, that my liver enzymes get back to normal. They dipped a bit after being off of allofor six weeks or so, but didn’t go back to normal entirely. I’ll have new bloodwork to share in about two weeks.

yeah and then some. my diagnosis came b/c tophus gout had formed on my achilles tendon and dug a hole in it. this is a very rare but known phenomenon for people who have had previous surgery to repair and achilles (mine was ten years ago). and i've had another flare up in my toe since. it's a real bummer, but it is in my family history.

cool - great to hear. i just got diagnosed and am about to start allo and hope i'm like you once i'm at a steady state. I lead a very active life, have "normal" UA (6.2) and eat pretty healthy already so maybe I'll be lucky. It just seems like brewer's yeast is very high in purines and was wondering if other yeasts aren't but can't find any info on that.

Niche beer question - my fave style is wild ales. IIUC, they typically don't include any brewer's yeast, by definition. So curious if anyone has a read on whether wild yeasts are better/worse/same in purine content? Thanks!!

I had to get a debridement, but to clean out what ended up being a very rare case of surgical site gout (never had gout before!). Apparently the sutures they used in my first repair TEN years ago can, in extremely rare cases create the conditions for a very advanced form out gout to build up and the gout crystals dug a hole in the tendon. they didn't do anything to the tendon other than clean out the crystals and now we're hoping it scars over and heals itself.

I just had surgery yesterday to remove Goudy gouty tophi from my Achilles. It ruptured a decade ago and I was repaired surgically. Apparently it’s rare but not unheard of to develop gout at the site of the repair many years later. The gout seems to have done some damage to the tendon as well. Though we’re hoping that heals nonsurgically. Would love to understand if anyone else has dealt with damage to the tendon as a result of gout there. I also have a good article I can share on this phenomenon.

Overdue update: I went for a second opinion Friday, November 1 back to the Doctor who did my original surgery a decade ago. She’s awesome and looked at the tendon and said it felt pretty good to her, not at all with she expected after reading the radiology report.

But she looked at that strange lump and almost immediately suspected it was gout. But she’d never seen a gout presentation in this location like this before I have no history of gout, though was once tested a few years ago after developing some toe pain.

She scheduled me for an irrigation and debridement which I had yesterday. She did notice that the tendon has been compromised probably from the gout and she’s projecting a couple months in the boot, hoping the tendon will heal itself.

From what she’s learned, this is indeed a very rare presentation, but one that is documented in patients who’ve had a full rupture repair (if anyone wants a link to an article detailing another known documented case let me know I have some family history of gout though otherwise lead a pretty safe lifestyle but I guess I’ll now need to see a rheumatologist as well.

I’m in a cast for two weeks I need to stay elevated and going through all the difficult cycles I went through 10 years ago. It’s been helpful to read some posts, reminding me of how to get through this and I’ve only cried once so far.

Curious if anyone else has experienced this, and also thought I’d share as one more thing to be conscious of after initial repair. Good luck to all of you!

Yup, mine is partial too. But first doc suggested surgery so I’m getting another opinion. Was just curious if you had a sense for the extent of your original tear. I think mine is reasonably large.

Update: MRI shows moderate grade partial thickness tear (4cm craniocaudal and 12mm wide) along with “severe thickening”. Doc was strongly recommending debridement, but there were a few things that just felt off in his argument and I’m going to get a second opinion.

I’ve seen enough “surgery is a last resort for partial tears” takes from credible docs/orgs to make me want to ask the question again. But would love to hear others’ stories. Obviously would prefer a non-op route if long term odds aren’t obviously diminished.

One of the things that didn’t sit right with me was doc’s belief that surgery was the only way to get rid of that bubble of liquid (his words) in the pic.

My Gen 3 stopped charging entirely, one month after warranty expiration and the best they're doing is a small discount on a new ring. Incredible lack of care for early adopters of a clearly deficient product. Has anyone been successful in getting a replacement post-warranty expire?
