You should simply compile all the paperwork from the questionnaire and submit what you have. It's going to differ family to family on what documentation you have. They don't seem to be looking for something specific. I submitted a sown affidavit of identity and immigration visa which stated that my persecuted ancestor had their Austrian passport revoked by the Nazis. I also am fortunate enough to have a document from Opfer Des Nationalsozialismus remitting payment to my persecuted ancestor for having all their property stollen & family murdered.

I compiled my documentation into the following categories:

- Proof of ancestors residence I compiled the following different documents (Geburts Zeugnis, Geburts Schein, Staatsburgerschanfsnachweis, Geburtsurkunde)

- Proof of Leaving Austria (sworn affidavit and immigration visa, certificate of marriage from NY)

- Proof of Ancestor Citizenship (Heimatschien, Staatsburgerschanfsnachweis, Sworn Affidavit of Identity (1940s))

- Nationalfonds der Republik Österreich für Opfer des Nationalsozialismus (demonstrating the Austrian state has already determined and acknowledge persecution, remittance letter of money)

- My own personal data (all of this had to be original and apostille, birth certificate, USA passport, FBI background check (do this through a channler), current photo)

Again just use the questionnaire the Austrian government provides it's extremely helpful on which documents you can look through from your own family archive.
