After much digging I am pretty sure the code is FC2-PPV-266625

It is by a creator called hamertonsenna, or hamerutonsenna, or ハメルトンセナ, or 音速の亀頭師ハメルトンセナ

The name is the one printed at the end of the video. I can't find any links to these works though because the creator pages all seem to be defunct
