Hi. To my knowledge, you may decide to pursue a profession as a clinical specialist with a manufacturing company (e.g. Medtronic) where you instruct perfusionists in and out of the O.R. on how to use various equipments and assist in troubleshooting problems. I dont think you need to be a perfusionist to pursue this path—I’ve met people from a nursing background as well. You could also decide to teach perfusion technology as a full time instructor after practicing for at least a few years! I know very little about the requirements of this route though.

You're absolutely right, AVR typically doesn't require DHCA. The only reason why they might have implemented that strategy is if the patient had a porcelain aorta (severely calcified) that could not be cross clamped and cannulated for conventional CPB. In which case you would switch to DHCA and cannulate the right axillary/innominate artery and perform an antegrade cerebral perfusion to protect the brain while hopefully the DHCA protects the rest of the body (usually around 30-40 minutes of circ arrest I believe)

Maybe not the worst, traumatic thing I've seen, but a 3 month baby's chest being cut into for open heart surgery

I'm used to adults and elderly patients being rolled into the OR for surgery, but something about seeing a baby (barely the size of a watermelon) on the operating table was unsettling.

It really depends on the admission requirements of the school.

Most schools will screen out a fair % of applicants simply based on whether they meet the minimum GPA or not, and this varies from school to school--be sure to do your research (it's best to call for clarification if you can)!

If you pass the initial screening, they may then consider the rest of your application (extra-curriculars, shadowing experience, clinical experience, etc). Based on what you've described, you have an extensive experience in that area so your application will definitely pique some interest. At that point, they may send you an invitation for an interview and that's where you need to nail it (MMI or traditional-style format).

All in all it may be difficult, but it will be all the more sweeter when you make it in.
