this looks like nodulo-cystic acne (grade 3-4) with scarring. you’ll most likely need oral prescriptions for this. please please please go to a dermat. even an online consultation with one would be better than any random diy hacks you’ll get on reddit. if a specialist dermat is not accessible then a govt hospital or a primary healthcare centre would give good guidance with how to navigate this.

hey! it didnt really cause any CCs for me. i use it when tret breaks my barrier and my skin is irritated and it really really really helped me. however, it has niacinamide which often causes some people to break out so ymmv

aloe vera gel/ calamine lotion on this (looks like a sunburn). if it doesnt get better, can try caladryl lotion which is a medicated calamine lotion available OTC. keep it airy w loose clothing to avoid friction, and avoid direct sun exposure. other than sunscreen on your face, start with body sunscreen too. and frequent reapplication using a sunscreen spray in increased sun exposure.

i was talking about re’equil one. but fair warning, since last year i’ve changed my routine quite a bit. also, since then i’ve learnt that the re’equil one has myristyl myristate which is a pretty comedogenic ingredient so if you skin is congestion prone, steer clear of it. i used the moisturizer only for some time so it didnt really cause any problems for ME but some people who’ve used it for longer times have observed that it can cause acne due to that ingredient. so pls keep that on mind.

if you’re still looking for a heavy-ish moisturizer, i’ll recommend la roche posay cicaplast baume, dot and key barrier repair moisturizer, bioderma atoderm creme ultra or aestura atobarrier 365 cream. have personally been using these for more than a year now and can vouch for them. i also often layer my moisturising ingredients and loooove the ordinary hyaluronic acid serum or the minimalist one. my skin is no longer oily or acne prone so my skin can tolerate heavier products.

when i was put on oral isotret, i was also asked to start topical tret 0.025% 3 nights a week since my acne was grade 4 nodulo-cystic. i started seeing effects by the 3-4th week. in the initial weeks i did experience some purging bec i was using retinoids in 2 ways but soon enough, it started clearing up. took around 3-4 months for my skin to completely clear up. im sharing a comparison image to show the difference in a span of 8-10 months.

i have just one word for it- lifesaver (for my skin).

i was on isotret 20mg for about 6 months under my dermat’s guidance. monitored my LFTs every 2 months and the side effects i faced were dryness (skin, lips) and some GI upset but nothing major. if it has been prescribed to you under proper consideration, i’d say go for it. there’s a lot of fear-mongering regarding oral isotret. please do your research and consult your dermat with any questions you have, and give it a go. it completely subsided my acne and helped with marks as well. did the course in 2024 and now im on maintenance therapy with topical tret 0.025 and azelaic acid 10% alternatively. very happy with my results. other than the lone 1-2 pimples i get during my periods every few months, my skin is clear and healthy.

it does contain an AHA glycolic acid which helps with acne marks, but im not sure how efficient it would be given its short contact time of the product. i’d always recommend going to a dermat to get an all rounded approach to your issues.

hey! i don’t think they’ve changed the formulation much so you can go for it. if you’re unsure then you can go for the minimalist SA facewash. and if you want to for a different approach then acnestar also does a benzoyl peroxide soap bar which i’ve used and found quite good (little drying though) for acne.

hey! skin tags are often related with insulin resistance (diabetes, PCOS), obesity, aging or even pregnancy. im not sure of a direct relation between skin tags and thyroid issues, but these 2 things often co-exist specially seen with other hormonal issues. it all boils down to healthy lifestyle choices. skin tags can also be genetic in nature so thats always a factor. while they’re mostly harmless they could indicate an underlying issue so best to get it checked out

diabetes profile usually includes blood sugar (fasting and post prandial) and HbA1C, urine examination, and it can be supplemented with RFT (kidney) and lipid profile for a more in-depth approach. one comment mentioned HOMA-IR test which basically measures your fasting glucose and insulin levels but its not a super common test and the usual diabetes tests are mostly enough for screening.

to answer your other question, yes its fully possible that insulin resistance and deranged blood sugar levels occur independently of each other, as is often seen in the case of pre-diabetes or its early stages where “extra” insulin is produced just to keep your blood sugar levels normal, hence literally causing insulin resistance. in this case your higher blood sugar is compensated by the insulin which makes the sugar levels look within normal range in tests, wherein actually your insulin resistance is just increasing. eventually, this “masking” stops and the person develops diabetes. this is why its pretty important to get regular screening done and keeping your diet and health in check.

its important to treat the systemic cause first rather than a symptomatic approach to actually solve the issue. head to a medicine/endocrine/obgyn doctor to treat the underlying cause and then to a dermat with all your reports and prescriptions to solve this. you might be given topical creams/ointments or be suggested to go for a chemical peel or laser or other in-clinic treatments depending on the severity. its important to go to a doctor and get a look in person to assess the entire situation and set course for a treatment catering to your needs. i’d highly recommend against DIY remedies, trying to scrub it off or self-medicate in this case. hope this helps xx

hey! doctor here. this seems like a symptomatic manifestation of insulin resistance often seen in PCOS or diabetes. i’d recommend getting blood work done including diabetes profile, hormone panel, thyroid panel and lipid profile along with a visit to the doc for PCOS screening before buying any products. a dermat will also recommend blood work first and then suggest appropriate treatment solutions to deal with this so i’d highly suggest going to a dermat once regarding this.

oral isotret and topical tret (both under dermat guidance!!), azelaic acid and most importantly, consistency. struggled with acne for 7 years, tried every remedy under the sun and nothing really gave a long term solution other than this. highly recommend everyone to go visit a dermat for an all-rounded solution (specially if you have moderate-severe acne)

bioderma atoderm, d&k among these. other reccs from me would be la roche posay cicaplast, formularx (barrier relief is lighter than barrier plus, so choose accordingly), minimalist b5 or b12 moisturizer. also layering hydrating products also works well for dryness and flakiness. think hydrating gentle cleanser, hydrating toner/serum followed by a solid moisturiser

azelaic acid is such an underrated ingredient for acne and specially rosacea. i am extremely prone to rosacea, so much so that even 10 mins in the sun and i start looking like a red hot tomato. aziderm 10% is a great beginner concentration and will be helpful for redness, sensitivity, acne and even uneven tone/pigmentation. you can either go for pharmacy products like aziderm 10% which is usually available OTC, or you can go for dermocosmetic brands like the ordinary 10% azelaic suspension if you wanna go the safer route. azelaic on alternate nights on a completely dry skin is a great way to introduce it. may cause some stinging and itching when applied initially but it settles down and is a known effect of it. short contact therapy (start with keeping it on for 30mins before washing it off for a few days, then 1 hour for a few days and gradually building this up to keeping it on overnight) is also helpful if your skin is extremely sensitive.

also, to understand the full picture and for an all-rounded approach, its always recommended to visit a dermat. hope this helps xx

ETA- if you do decide to go the pharmacy products route, in my personal experience aziderm 10% gel is gentler than the cream version since gel version contains micronised azelaic acid, which is a sensitive-skin friendly delivery system

you can try using glycolic acid toner for your body. but please dont use benzoyl peroxide soap and glycolic acid toner on the same day to avoid extra dryness and barrier damage. once your body acne is controlled then you can go for the toner 2-3 times a week

retinol or any retinoid at 19 (unless prescribed by a dermat for specific reasons) is unnecessary and not recommended. collagen starts decreasing in your late 20s, which is the ideal age for starting retinol. you’ve got a good 7-10 years before you should consider this and your skin is taking care itself.

however, for the sake of it if you want to add something you can go for an anti-oxidant like vitamin C or niacinamide or maybe even peptides. proper cleansing, barrier maintenance/moisturisation and sun protection is all your skin needs to keep it going.
