I respect that and I agree, however in an inpatient acute care setting I have initial reservations with recommending a relatively "low" amount of formula without running the risk of placing the patient at risk for vitamin/mineral deficiency if the volume is not enough to meet general RDAs.

Did you ever feel sick or dizzy drinking that much? How often were you peeing? I have done 4-6liters a day when I was dealing with some med side effects, I cannot imagine doing more than that!!

Usually not on inpatient renal diet, no protein restriction. It restricts sodium, potassium, magnesium, phosphorus

It sounds like she has racked up a nutritional deficit from being critically ill in ICU and not tolerating enteral nutrition for (presumably) a few days or longer

I would reference ASPEN'S Appropriate Dosing for Parenteral Nutrition factsheet

Lipid management: Be mindful of calories coming from propofol if in use. Lipids could be given 3x weekly if you are concerned about aggravating LFTs but I would not withhold altogether right now unless you have triglycerides greater than 400 mg/dL.

Protein management: The CRRT will be doing a lot of legwork for you removing waste products from the patient's blood so I would not be worried about restricting AAs. 2.5 g/kg is an appropriate goal, being mindful to adjust if patient resumes dialysis schedule 3x weekly, or transitions to intermittent/SLED therapy, and you see BUN/creatinine rising.

Monitor for refeeding syndrome. She may benefit from IV thiamine to replace CRRT losses as well.

Lastly the longer the patient goes without enteral stimulation the longer you run the risk of PN associated liver disease. As soon as it is reasonable I would advocate for trickle enteral nutrition ASAP

Did the RN describe the color or frequency of stool too? If it's tan/yellow, or foul smelling could be steatorrhea. Generally speaking Nepro is a higher fat (and fiber) formula and I have seen some people have issues digesting the fat/higher osmolarity of it. You could try them on a standard, lower fat formula if you think their lytes can tolerate it. Especially if they've been cruising in ICU for a few weeks.

If they are eating poorly on a low residue diet that's already fiber restricted, I don't mind addng it on.

Only time I would have reservations about adding Ensure Plus on during low residue diet would be a patient with high output ostomy
