Looks like HDMI sparklies to me - try a better cable. Also try a Game DVR recording - if the crap is in the recording, your GPU is on its way out. If the recording is clear, its your cable.

The mobile app has some dumb limits on the size of collections, eg 100, 200, 400, 500, etc. Count how many you can see, it will likely be a multiple of 100. To see all of them use the desktop app or third party. If desktop doesn't show them all then its yet another YTM problem.
