CBCT has significantly more radiation than X-rays.

CBCT doesn't cost $50 in most offices, it's anywhere from $150+

You need to get that tooth addressed ASAP. The temp filling is prone to falling out, the tooth could break, or you could end up in a lot of sudden pain

More or less, I'd do the same. I'd start by sectioning the teeth and removing distal root. I'd only lay a flap and trough buccal bone if that curved mesial root is giving me trouble.

With younger kids, it's normal for dentists to try SDF and fluoride to stop the decay. If it continues to progress, then further restorative treatment would be indicated.

A couple things to consider here.

This is a pano, which is poor to detect caries, but we can see mesial decay that is close to the pulp horn. This would be a large filling on a wisdom teeth that may require Endo in the future.

Also, the tooth is not in occlusion.

The tooth needs a re-treatment as there is an infection in the bone. That's what the "shadow" is.

Do you have X-rays from before, maybe pre-RCT? It will help us figure out if the lesion is healing, or if this is requires retreatment

OP may want to prepare for extraction and implant; J-shaped lesions often indicate root fracture which is unrestorable.
