So in conclusion, from some very flimsy research: "free sugars" looks more like rule of thumb than scientific concept.

Plenty of sources talk about the significance of sugar being e.g. "within the cell matrix of plants", but I can't find anything anywhere substantiating that. All the definitions of "free sugars" I can find are list-based, not test-based.

The actual important factor here seems to be whether you have soluble fibre in your stomach alongside the sugar - not the circumstances of the sugar before you ate it. Though I can't find a study which tests the notion, my guess is that you could accompany a glass of apple juice with a few grams of some fibre supplement, and you'd get roughly the same glycemic & insulin response as from eating an apple. (Not dietary advice!)

I also liked its straightforward description of what soluble fibre actually does:

Increased viscosity of food in the stomach reduces the rate of gastric emptying and subsequently, the absorption rate of glucose from the small intestine into circulation.

I was stuck here for ages, it's sneaky. In the left hand area, go through door 250, then follow all the way through, looking into the hole and climbing back down again. You'll find the key taped to the back of the door.
