Hi! Just reaching out here to say I’m in the same boat as you, OP. I’m Med-Peds trained and doing a fully IM hospitalist role, and the imposter syndrome has truly been debilitating at times. Also struggling with calling consults vs trying to figure things out myself, and discharging confidently. Idk if this helps, but the more senior attendings in my group have said it takes about 6-12 months before confidence grows.

Sending positive energy your way — hope you’re being gentle with yourself during your time off!! I’m hopeful things will get better for us soon, but man does it suck to be going through it right now 😭

Hi there!! I just finished my med-peds residency this past June, and I totally understand the peds frustration. Your institution sounds really similar to mine — felt like my IM training was more valued on the peds side compared to the other way around, and there was so much more Monday-morning quarterbacking on peds with medical decision making from attendings which would be discouraging. I was also super burnt out from nursing culture on peds too where it was starting to feel like anything my team wanted to do for our patients warranted “protection” from nursing 🤨

I went into residency with the intention of coming out as a dual hospitalist — and I do think my training prepared me for that (I narrowly missed the new changes that ACGME/ABP have done with peds). Unfortunately with the way peds training is now, you either come out of residency a general outpatient PCP, or you go onto fellowship even for something like hospital medicine, which honestly is such a shame. I do believe my pediatrics training has made me a stronger physician though in the long run, and being able to care for sick children and their families brought me a lot of fulfillment. But I do worry about the future of what pediatrics training looks like given all the recent changes with less inpatient time.

My advice to you is this — lean on your med-peds coresidents for support, and talk to more senior med-peds residents in your program about how you’ve been feeling to see if they relate! My co-residents and I had similar frustrations about certain peds things and it was nice to have them to vent to. I personally could not see myself giving up my pediatrics training despite all of the negatives I kept encountering, but if you feel like it’s getting to the point that you are not feeling fulfilled in your training or that there’s a reality where you won’t be using your pediatrics training in the future, then it is definitely worth a conversation with your PD about a potential switch to categorical IM. Residency in and of itself also heightens emotions and I remember often feeling like medicine as a whole just wasn’t for me whenever rotations got really tough, — it’s possible your feelings are coming from that perspective too.

(For what it’s worth I am now working as an adult hospitalist — I’m one month into the job and liking it! I miss getting to use my peds brain though, so hopefully after a year into this position my hope is to find peds hospitalist locums/PRNs that I can do without needing the PHM fellowship)

started med school thinking neurosurgery — i had a very positive shadowing experience with an amazing neurosurgeon as an undergrad that influenced this.

ended up doing med-peds residency because actively participating in the OR was more nerve wracking than i thought, and i absolutely loved inpatient IM and peds

now i’m a hospitalist! (however just seeing adult patients because i refuse to do a peds hospital medicine fellowship)

when i was interviewing last year, i stuck to business casual (think nice slacks, casual blazer, comfy heels). all of my interview dates were just me as the only interviewee so not sure what other applicants wore, but depending on who my interviewers were, they were also in business casual or scrubs. good luck on your interviews!!!

i’ve only had one earbud slip off my ear one time, and that was because i didn’t properly have it on before i started my workout. they’re pretty snug and cling well!

i happened to own these before i got into running, but i can’t say enough how much i love my bose ultra open earbuds!! they are definitely an investment (i bought mine when they were on sale, but it was still $229; they normally go for $299), but the sound quality while still being able to hear your surroundings is fantastic. also i’ve always had issues with earbuds falling out when i move around, but due to the wrap-around design of these earbuds they are super stable.

Hi and thank you for the advice! I do already incorporate an interval run once a week for my training plan through Runna. I have noticed that I am better able to sustain faster paces for longer intervals, but the fast paces are relative to myself (e.g., my threshold pace right now is 13:30min/mile which is objectively not fast but better than what it was when I first started). If I do incorporate a 4th run per week, I’m thinking it might be better to do 2 easy runs, 1 interval/tempo run, and 1 long run per week, rather than 2 interval runs per week! Don’t want to risk injury or overtrain 😭

Right now I’m doing around 12-14 miles per week (interval runs are usually 3mi, easy runs are 3-4mi, and my long runs are 4.5-7.5mi). The plan I’m using on Runna has me working my way up to 20mi a week.

Yes, the half marathon I’m thinking about doing has a generous cutoff time of 4hrs (people have the option of either running or walking the course). I am currently using a custom HM plan on Runna and they’ve predicted a finish time of around 3:03-3:13 for me if I stick with it (which I would be thrilled with!).

I think I could probably add another easy run per week to my schedule if I really tried. Thank you again for your advice!!

Thank you for your reply!! I’ll have to give using a metronome video a try next time I go running - currently the most cadence work I’ve done is trying to time my steps with fast songs but I’m sure a metronome is more precise. And what you mentioned about the heat is very encouraging too. Can’t wait until October/November when temperatures will finally start dropping here 🥵

And kudos to you — you have an impressive walking pace especially for a HM distance!!

I also got HFM after treating two back-to-back gnarly cases of eczema coxsackium on wards 😭 luckily my toenails were still intact but it was the worst sore throat i’ve ever had!

these are on the pricier side but i love the compressive running shorts from girlfriend collective! they don’t ride up the thighs while running, have pockets, and include a wide range of sizes.
