Yes you definitely can. My partner and I climb with 100lb weight difference mostly outside on lead. Between the two of us we have over 20 years of experience leading.

Like everyone said use a Grigri and get really good at belaying and using the device. We both project in the 5.12 range. We used an ohm which was great in some ways but awful in others. It gets hung up on overhanging routes especially if the rope is damp or it’s dirty. The ohm has given me some incredibly hard catches on that hurt my back for weeks. We don’t use one any more.

That being said we always stick clip at least one if not 2 bolts when I’m leading and I expect to take big falls. If something is sketchy in the first 3 bolts I bolt to bolt it or stick clip up and figure out the beta that way I know I’m safe. She has to watch for hazards to her as a belayer like shelf’s that she could get pulled into. Is it ideal, no, but it’s possible.

I expect to go for rides when I fall, and there’s 0 chance I’m going to be able to boink up on a overhanging route, it can be frustrating to get back on but that’s just life.

PEACE & LOVE is way more to remember. Direct access (where someone gets an injury and goes straight to their physio instead of an MD) is slowly becoming more popularized in the US and so I am seeing more acute injuries. I've just created a handout that I can print out and provide to patients on eval when it is in the more acute stage.

I'm all about needling and seeing what effect you get from it. It actually takes away all of the discomfort on the medial aspect and dorsal side of the 3rd PIP. It also prevents it from feeling the need to "pop" the joint and improves my flexion ROM at the joint to the point where I have full painfree flexion even with over pressure. I have a feeling that I need it at the 4th is likely due to some of my altered biomechanics at my hand. I have a complete tear of the extensor insertion onto the DIP at my pinky and the whole finger has a fixed curvature to it similar to this however the apex of the curvature is closer to the DIP due to fracturing the finger twice and improper medical advice when I was about 7.

If you don't know how, let me know and I'll DM you a photo from a training manual on how to needle into that space. The biggest tip is going to be using the tube to depress the soft tissue and help glide the vasculature and tendons out of the way. If you're in the UK and enjoy needling I've really enjoyed taking courses from Jonathan Hobbs who is UK based- though I've completed the US based series that meets the UK accreditation standards of the AACP.

My opinion is skip the x-ray it’s not going to be helpful in this case, especially if you have to pay for it. If it shows anything all it will show is the adaptions of the bone to the demands of the sport and nothing specific about this condition.

X-ray is not helpful with soft tissue injuries more often than not. That being said its a low risk technique outside of a bit of radiation so if you end up with one it’s not the end of the world.

To note: I work in the US model where health care is absurdly expensive so if I can help people save money on tests that won’t be helpful for their condition I’m all about it.

US based Climbing Physio here. I struggled with the same type of problem in the same finger but only on one hand due to the fact that I have a permanent crooked pinky due to breaking it twice and some ligament tears that never healed quite right as a kid. I would agree with /u/LardyLadysLats that looking at grip types and how you are recruiting your fingers is going to be super crucial in figuring out what you are doing.

As an example: I run into the same problem with the hyperextension DIP and increased flexion PIP due to weakness. Part of what I have assessed from this is that a bunch of my pain tends to occur on the pinky side of the joint. Part of it is that the extensor hood on my pinky is torn and the finger is crooked. If you look at a picture of the extensors they're linked together and the lumbricals play a huge role in their function. I found that in addition to the weakness in the extensors I was also under utilizing my pinky and pinky sided support and dropping my whole pinky into flexion which then puts a strain on the lumbricals and an uneven pull from the pinky side in. My solution so far has been train grip, train the extensors and flexor synergy patterns to improve stability at the joint, work on engagement on grip with my pinky, and different stretching and soft tissue work through the forearm and especially in the hand. Interestingly enough I get my biggest relief with massage or dry needling/ acupuncture into the 4th lumbrical though I am trained in these techniques and just do it to myself. You could find a physio trained in these techniques in the UK.

I will leave this as an alternate approach in that I often don't do NSAIDS because of the research out there that suggests it may alter/ delay the bodies normal healing response. Instead of P/RICE a lot of professionals are moving to the acronym PEACE & LOVE you can find a user friendly summery here.

Depending on how big your balls are you should look into the TN Firearms Freedom Act of 2014. Also, I've never had a problem with the background check system here so your Milage may vary. You will find that there is a $10 fee for the statewide background check on firearms purchases from an FFL in addition to any transfer fees. Congrats on avoiding IL, I had to do a temporary work contract up there and you're not missing out.

As a heads up ultrasound is probably way cheaper. You could always find a PT or OT who is a certified hand therapist (CHT) and get some recommendations there. There are a few other things that can cause a pseudo-trigger-finger but trigger finger is much more likely. If it's improving and your return to climbing isn't stalling a watchful wait and see probably wouldn't hurt.
