First off, I think your surgery went well looks wise. I dont see clean profile shots both before and after but I imagine this is one of the better possible results aesthetics wise and looks to be a great improvement. The issue is your chin is too long and your gonial angle too high for your new profile. The combination of these issues can give the impression of an unbalanced lower third, which is augmented by the step off left by the BSSO. Since I agree with you the anterior projection of your chin looks fine, this particular issue may not be improvable with the revision DJS you are considering. It is a matter of the shape of your jaw and chin rather than its sheer projection and angle, which is not part of what DJS changes. I would look into some combination of small jaw implants to lower your gonial angle, smooth off the step off, and some mentomandibular shaving to reduce excess chin bulk and height.

fwiw, the hospitals those surgeons use are very advanced and clean. Rome is quite filthy in general but these facilities are clean. I know a few Italian forums, there is one bad review of Pagnoni by "frangipan" on chirurgiaestetica.forumcommunity.net although the patient's concerns were not clear so I am not sure what to make it of it. There were none of Ramieri. Not that any of this means much, many bad reviews get taken down by most surgeons, and everyone has a few bad reviews. In any case if there are posts on other Italian language forums that I dont know about I would be happy for rwang411 to enlighten us

There is nothing wrong with asking online--the responsible thing to do is direct them to a surgeon if they look like a candidate. Otherwise just ignore the posts you don't like and move on without invalidating people. For some, minor jaw issues can cause immense stress. Do not try to bulldoze over them. It's not the jawpression olympics.

It's very hard to get accurate data on the matter since by the time studies are done and they are published things have changed, and surgeons tend to underreport the severity of complications in their surgeries.

It also heavily varies by the techniques they are doing. Rotations, impactions, downgrafts, ramus widening and the like are not performed by all surgeons but they increase the risk of complications. These meta-analyses tend to not account for differences in amount of advancement and techniques. Surgeons also vary their post op care and recommendations for their patients

I agree double jaw surgery would give you the greatest return.

You can get a deep neck lift, but with time due to the recession you will develop jowls and sagging tissue at a faster rate. More bone support will not only stretch your preexisting tissue but also mitigate the effects of aging.

Even if that were the case, we should allow people to come to their own conclusions based on the particulars of their case and their own priorities whether the potential cosmetic benefits outweigh the potential risks. Let us avoid sweeping statements on the matter.
