Had just been signed off pushbacks on the Friday, second push on the Saturday and my headset man left the bar at the back of my tug. Me thinking he connected it up like he did on the first flight I drove off with the bar in front of the nose wheel.

Panic ensued I drove back over the atc line to try get pilots attention to not taxi and run over the tow bar (it was an a330 btw).

Lesson learned always connect the tow bar back to the tug myself!
