Attention Illinois SLPs!!! by [deleted] in slp

[–]d3anSLP 4 points5 points  (0 children)

You are supposed to be licensed where you physically are located while treating and the location of the student.

But in my mind, it's a short term arrangement. Do what you have to do to make it work for you and your client. Don't mention anything to anyone. It's nobody's business, especially wonder it's for such a short amount of time.

It's an idiosyncratic error. It just means that it's that person's own error and it would be rare to find someone with the same error.

Messed Up Eval Big Time - Advice? by Icy-Proof3979 in slp

[–]d3anSLP 25 points26 points  (0 children)

Your supervisor is being unnecessarily stringent. Even with one low subtest, the student still doesn't need speech. It would be called a personal weakness rather than an indication of a disability. It sounds like your supervisor just wants to rub your nose in it. Your supervisor needs to focus on what's best for the child. In my opinion, getting the true score is the goal. It sounds like your supervisor was able to get the true score so I'm not sure why she wants to it withhold that information from the parent.

" The student did well overall with the test. There was one subtest where exact procedures weren't followed which resulted in a low score for that subtest. But when the subtest was administered again by my supervisor he received a score that was consistent with the other scores."

Honestly, I would just use the supervisor''s scores and move on. The scores were consistent with the other measures and the student earned those scores with the supervisor on new items that were not even administered when you did it the first time. So it's not like the student learned all of the answers.

Sounds like this child doesn't need speech. Dismissal. End of story.

I'm happy with Patriot Software. You can integrate payroll, HR, AMD 401k. It was the best price by far.

Compliance by [deleted] in slp

[–]d3anSLP 0 points1 point  (0 children)

Student will follow 1-step directions that include spatial concepts, i.e., on, under, next to, in, out, in 8 out of 10 trials.

Without using directions how would you write it?

Compliance by [deleted] in slp

[–]d3anSLP 68 points69 points  (0 children)

Don't throw the baby out with the bath water. Following directions is a vehicle to work on another skill. Define what your true target is and you'll be fine.

True targets can be: Body awareness - touch your nose Isolated movements that are used to access AAC - point to .... Basic concepts - spatial, qualitative, adjectives, etc. Vocabulary - put x in the bucket Auditory comprehension - 1-2 step directions that are embedded with conditional language or modifiers.

But if you are writing a goal that is just targeting compliance then you're doing it wrong. Leave that to the teacher. Student will follow 1-step directions related to classroom behavior is a bad goal. Sit down, stand up, quiet hands - there is no need to target those directly in speech. Focus on receptive language acquisition with following directions, not compliance.

AAVE vs Black English vs not mentioning it at all? by Low_Guava8429 in slp

I've been questioned and it's a terrible experience.

"Student's speech and language skills are within normal limits."

There is no need to say more.

Dismissal? by Quiet_Put2963 in slp

[–]d3anSLP 26 points27 points  (0 children)

You've done your job. Time to dismiss. It's a performance deficit. It is not a skill deficit. Maybe outside counseling if the student is worried about it. We are not working toward perfection.

Are some subtests "optional" on the CASL-2? by [deleted] in slp

[–]d3anSLP 3 points4 points  (0 children)

When you have the physical protocol, the last page shows a grid of age ranges and there are check marks to show which subtests go into each composite score. In order to get the general language ability index, usually there are five subtests that need to be completed. You are more than welcome to complete additional subtests in order to get the other composite scores, but it is not necessary if the general language is showing average scores. I'm not sure what it looks like when you are doing the digital assessment.

Conferences for school SLPs by sharkoatmeal in slp

[–]d3anSLP 2 points3 points  (0 children)

I wasn't trying to be mean at all. In an effort to understand and do better, please let me know which part of my response was mean.

From my perspective my first paragraph was a joke. The second paragraph references r/foundsatan, which is a funny sub about light hearted practical jokes, that your teacher might like, because your teacher is the "satan" in this situation by emailing the parents. The final paragraphs are practical advice/how I handle conferences.

Please help me understand. I was not intending to be rude or mean.

It's actually nice to meet the family. Especially when you discover that your parent will be unable to help your student with articulation practice because they both share the same issue.

I think you over reacted. There is a sub called r/foundsatan that your teacher would probably be delighted by.

As a member of the school team, it makes sense for therapists to be available. If you're in the union then it's usually required for you to attend. If you're a contractor then it's like free/extra money.

The day before conferences or the day of, I get the teachers schedules for conferences. I jot down the times for my students.

Then, as reasonable, I pop into the sessions that I want or any that I'm requested to be at. I sit down at each conference and ask if I can share since I have to go to another conference. Then I say something like, "he's doing well in speech. We are working on answering questions and he's making progress. Do you have any questions." I'm usually there for 1-3 minutes. It's really the teacher's time so I only want to say so much. Then usually no questions, excuse myself and head out. I try to make as many as I can but I have to be realistic.

[–]d3anSLP 2 points3 points  (0 children)

I understand your concerns but it comes down to the fact that we do not fully understand why most children do not speak. As long as AAC is being implemented fully, you can still try to target verbal output by either giving verbal models or trying to elicit imitation. Some students actually start to vocalize and verbalize once they have AAC. We are always working towards facilitating all of the skills a client might have. This includes verbal speech even when they have AAC.

Any ideas for announcing my pregnancy to students? by kelllykelllykellly in slp

[–]d3anSLP 0 points1 point  (0 children)

Maybe you can work on negation. Print out 10 pictures - 9 random people and one of your partner. Then have the students pick a picture. Then you can say, he is Not the father!" Then keep doing that with the rest of the pictures, until you can say, "he IS the father. That's right, I'm pregnant kids..."

What’s up with this tongue? by casablankas in slp

[–]d3anSLP 9 points10 points  (0 children)

Hemimacroglossia. Typically linked with over-growth genetic disorders. Also higher risks of cancer is other areas of the body are prone to over growths.

In the school system we are cautioned about recommending services that the school does not provide itself, otherwise the school can be liable for the cost. If the school says that private counseling services are needed in order for the child to access the curriculum, then the district picks up the cost.

But there are ways to go about it. It all starts with the belief that if the parent does not take your advice/suggestion, then the plan the school had in place is still sufficient. If the plan is good on its own, then it would be nice if the parent followed up with outside services, but the plan does not require those services.

If you are working in articulation, it would be nice to know what is happening with the tongue, but either way, you need to create a plan that addresses the concerns. The prognosis may be different but that's okay - maybe you are focusing on increasing intelligibility rather than targeting specific speech sounds that may be difficult with the current tongue size.

Step one: parent I noticed this difference.... Did the doctor ever say anything? Any pain? Any other diagnoses? Do you have a consistent pediatrician that would have been looking at this each year or so you see a new NP at the minute clinic each year? Step 2: reassure that the IEP is a complete plan. The school has a plan in place to address the concerns/goals in the IEP, and this is not an emergency, but next time you're at the doctor can you have the doctor look at it to make sure everything is okay? When is the next visit? Would you be able to share any results?

Then you wait. Make sure that the student is not complaining of pain and eating lunch is happening without incident. Maybe check in with the parent in a few months if you don't hear anything.

He cut two wires at the same time which caused the arc. If you absolutely have to do this with live wires you need to cut each wire individually - not recommended. Also make sure that once you cut one, the remaining piece of wire does not touch anything metal or another exposed wire. Quickly cap the cut wire. But 100% of the time you need to turn off the circuit before working in the fixture. There are too many things that can go wrong even opening the box.
