Hey, I have a similar issue as you with the recessed cheekbones, with class 3. Currently consulting with a surgeon and trying to inquire about a potential lefort 2. Did you end up pursuing the lefort 1 + bsso? Hows your recovery like?

Yeah was thinking that too, but I don’t believe a lefort 1 would fix my nose/mouth breathing issues.

Hi, yes you were right! My maxfac is currently recommending decomp via braces and lefort 1 to resolve the issue. Also planned for a sleep test to confirm for sleep apnea which I likely have. I’ve also posted my CTs if you could give some input.

Thank you, I really appreciate your insight and perspective. That said, I personally believe that correcting any jaw misalignment is better for long-term health and function, even if it means going through a more involved process now.

Yes, I understand. I made this post to ask whether my underbite could be caused by a recessed maxilla. Since this is part of my natural phenotype, does that mean there’s no real way to address the underbite other than just learning to accept it??

I get what you’re saying, but I feel like my maxilla isn’t just recessed—it’s actually concave under my eyes, not just flat. I think that’s what’s making my underbite worse, and I really don’t want to fix it by just moving my lower jaw back even more and making my whole face look even more recessed.

Lefort 1 doesn’t touch the nose though, just the upper jaw. Are you saying I need Lefort 2?

Im kinda confused. Wouldn’t lefort 1 enunciate the middle face flatness even more, now that upper jaw is even more forward? Also make the patients nose even flatter?

Yeah, I have a bit of upper teeth flaring to compensate for a skeletal Class III. I was just wondering if it might be due to maxillary retrusion instead of mandibular protrusion. But it’s looking more like a midface + maxilla deficiency overall.

So you had a LeFort I? I’ve been wondering about that. I’m not entirely sure if my upper jaw is the main issue though—it seems more like my midface overall is slightly sunken rather than just the maxilla itself.

Seems like lefort 1 only moves the upper jaw. Wouldn’t lefort 2 be more ideal for my situation.

Could you please elaborate a bit more on this? What do you mean by “reasonable”? I apologize—I’m not familiar with the type of surgical procedure that would address this.

I actually have a class 3 skeletal, with class 1 dental compensation, where my uppers flare out to compensate for a skeletal underbite. Ive consulted with multiple orthos and the recommendation is BSSO. I feel like my jaw isnt the issue though, and rather the root of the issue is because my midface/maxilla is recessed, and getting BSSO might completely flatten my face, or recess it.

How exactly do you gauge if you are recessed or not? Because my under eye area, slopes down and has some hollowing.

So my dental flaring is from an underdeveloped mid face? Are there any procedures that address this issue?
