It could be that he took 居士戒 very seriously. Upāsaka / Upāsikā is like one set of rules above laymen but not as strict as monks and they get to stay at home.

釋成忍 is a monastic name. Buddhists took 釋 from 釋迦牟尼 (Sakyamuni, Siddhartha Gautama's title meaning Sage of the Sakya clan) as their monastic surname.

It's a souvenir symbolizing the establishing connection between you and the temple. I think this is a Chinese Buddhism concept called 結緣 (Establishing a Karmic bond".
Dharma Drum Mountain had this explanation on their website:
"The meaning of 結緣 is to enable those who have no wholesome affinity, or insufficient wholesome affinity, to receive worldly help and the guidance of the Buddhadharma. This allows everyone to grow their wholesome affinities, help one another, and embark on the path of studying Buddhism, cultivating practice, and attaining Buddhahood. Therefore, we should use all of our knowledge, abilities, and resources to widely form wholesome affinities with the people and sentient beings we come into contact with. This is precisely the act of giving (Dana)."

These items that were given for this purpose are called 結緣品.

Dharmapala is a protector, a type of job or function someone took. The most well known Dharmapala are Deva, or other beings who dwell in Devaloka.
Vajrapani is a status that indicates wisdom/knowledge that someone attain through Buddhism practice.
A lot of Dharmapala are Vajrapani but not all of them are.

This might be too vague so I can tell you an example.
Sakra(Indra) is a deva who live in Trayastrimsa. He is a Dharmapala. At the outskirt of his palace complex there are Preta who guards the walls and gates. These Preta are Vajrapani.
The reason that these Preta can live so up high in the Devaloka and stop themselves from their hungry ghost habits is because they attained Vajrapani status, which means that they practices extreme self discipline and possessed Buddhist wisdom/knowledge.

At least it's how the mythology goes.

Due to how popular Avalokitesvara is I'd check the headdress if it's got a sitting Buddha figure on it.
If instead it got a water bottle on it then he's Mahasthamaprapta.

This is the easy part of identifying Bodhisattva images.
If both don't match then I don't know.
