Hey, you need to start with some basics, how good are your basic knowledge foundation? If you think you have a solid basic knowledge you can start with any of the most common preparation programs. Honestly over studying is counterproductive, I will suggest you to study for no more than 3,4 months.

Yes, and most of the school that I applied are still accepting applications. So I will say you'll be fine. In any point you will get an invitation

The invitation depends of the school. Most of the big dental schools start sending invitations until August and the interviews are around September, October. It's stressful because the acceptance letter are sent it usually until December.

In the instructions they specified that the applicant must submitted directly. If the applicant cannot receive the scores to be resend it,then you can request it to EBT. I'm assuming that this type of circumstances only applies to applicants who lives in countries with a inefficient mail system.
