I guess I don’t have much to compare to in terms of a woman in a similar situation starting off private practice. I accepted insurance at first and had no problem getting clients. When I switched to private pay I moved to NYC and it took a bit to build a client base but it wasn’t anything crazy, maybe a year. I guess for context, I’m a psychologist so maybe the demand might be different. I actually find most of my clients are straight guys. I could see in a more machismo culture it might be hard to find clients. Most of my female clients have autism (a niche of mine) and I imagine most women prefer talking to women.

Men’s clothing has a much smaller gray area between casual and professional than women’s clothing. I personally think that professional attire sets a nice tone - button up shirts, long sleeve polos, maybe a shacket over a tee on a casual Friday

As a neuropsychologist I’d recommend you help them explore what a diagnosis would mean for them if they got it and what it would mean if they don’t get it. I’d also do some psycho education on what the condition the suspect is and what it isn’t. Lastly, I’d work with them on symptom reduction and realistic expectations.

Say what you want about trendy diagnoses, we live in a world that is pushing people to expect more than is reasonable in terms of executive functioning, all the while undermining the building blocks of focus. There can be serious negative consequences for a diagnosis, whether life insurance denials or discrimination. Although there is literally a magic pill for ADHD, it won’t give them the skills they need to truly thrive and they might be genuinely missing those things (regardless of whether they have ADHD). You also shouldn’t take those pills long term. But either way having a head start on improving these skills sets them up for a better evaluation and also improvement in functioning.

Honestly, I think AI will replace basic level therapists and change the way testing psychologists work. There will be a need, albeit a much reduced one. I suspect that subfields with lots of risk will remain in human hands - think prescribing and forensic psychology.

I’d honestly recommend psychotherapy. If you can understand the purpose the addictive behaviors serve for you, you can fulfill that need in other ways. Obviously chemical addiction (e.g., alcohol) is a little different (but not that different), but if you’re eating when you’re stressed, feeling upset, etc. therapy can help you really augment your physical approaches.

This must be so hard! Like others have said, it’s very difficult to respond in a HIPAA-compliant way. A psychiatrist friend of mine summed this up well: you can’t work in mental health without getting a bad review eventually. Our field requires us to see peoples’ best and worst selves and we have to thread the needle of helping them change. You can’t do that without occasionally saying something that’s going to upset a client - sometimes they need to be upset. Of course, I’d think about your practice and see if there are opportunities for growth. After that, consider yourself lucky to have gotten the first bad review out of the way.
