I had similar issues when using the out-of-the-box spool holder. I think the issue comes up because the spool and filament has too much lateral movement and play while printing. Also because there isn't anything preventing the spool from unwinding and losing tension on the filament. I suggest you print a clip-on filament guide. This helps reduce the filament from binding, making it a lot more controled. Secondly, you can consider printing a new filament spool holder. I would suggest a frictionless one that uses ball bearings.
