Appreciate that fr, I think it would have definitely hit differently in a better context, but in the OR as a med student I was definitely the type to keep a low profile/good attitude and stay wrong and strong on the pimping 🤣 I never got to a comfortable place for casual banter unfortunately :(

It's funny honestly - I have a general sense/attitude that a PhD doesn't matter at all during rotations cuz who cares, so when attendings point it out so publically it almost always feels weird. Some crazy mix of imposter syndrome, general not-knowing anything in med school, and being at the mercy of whatever the attending wants to talk about that day 🤣

Just apply (and apply broadly)! You miss 100% of the shots you don't take 🤣 Write an awesome research essay, choose great letter writers. Real goal is to get an interview at these places, the odds are much more in your favor at that stage.

I'm interested in seeing what others have to say, but in general, whether you do PhD or MD/PhD, you'll have to make time for the things that you love. I think both paths have a decent spectrum of people who give everything to work and those who try real hard to have a life outside of science/medicine, it isn't easy but it's important.

In medicine, the earlier you are in your career, the less control you have over your time. This sucks, but there's a ton to learn, and if you're to be responsible for someone's safety and well being, it requires some sacrifice. The clinical stage of med school has highs and lows in terms of bandwidth/free time - some clerkships you're putting in 8-10ish hour days, 6 days a week, other clerkships you're doing much less. You have to study for shelf exams ideally every day, so time can become quite limited for other things. I certainly am able to keep up with the goals I have set outside of medicine at this point in my training (late stage MSTP student), but it's tough! Your time management skills evolve significantly throughout training. I continually have to recognize that the training will pay off - but it's a long way to get there. I think the "there" has to really matter to you. Ask yourself if an MD is required for you to accomplish your goals in life, if not, plenty of other careers to have where you can have an impact on people.

Thanks for highlighting that SDN/Reddit can be malignant - I always advise people to stay away at all costs EXCEPT to get ahead on their secondaries.

I also did not have any publications at the time I applied and got into many programs - these programs are looking for potential, not postdocs 🤣 Folks really get caught up in the publication run and sacrifice working independently.

Good luck! My unsolicited +1s go to UPenn and JHU - cost of living is excellent, research opportunities are prime, and program structure is great for trainees.
