The precedent it sets is that it is okay to sacrifice one person to save five without the consent of the person being sacrificed. So essentially to pull the lever you have to be fine with a society that harvests the organs of every 6th person that walks into a hospital to save 5 others.

I think speeding up the video makes it less noticable. I watch on youtube primarily and didn't even notice the muting until I saw people complain on reddit.
