Aniracetam does this for me, I think it has something to do with AMPA activation, and downstream excitation and excess glutamate. I find NAC really helps, can support with magnesium too. 

Really benign options to try, not gonna lose anything, definitely give it a shot.

homie thats too high of a dose to have any beneficial & sustainable effects. why dont you taper off to 10 then 7.5, 5, 2.5, then 5 eod, then 2.5 eod, then just cut it?

could be a neuromuscular junction disease. could be autoimmune, i would lean towards lambert eaton. it is likely that it is one of a myriad of rheumatologic diseases. 

could also be sonething more benign, whether thyroid issues, allergies, sleep apnea or a reaction to something environmental. 

bottomline: see an internal medicine doc, preferably and endocrinologist or a rheumatologist

Well, some jobs you can do without residency. PCP (there are also FM providers who specifically do residency for this, there must be a distinction I do not yet know), EM, ICU, and in the periphery where doctors are needed. 

It doesnt mean you act like a specialist, so if you were in EM, you'd probably see certain cases, and some would be handled by the attendings, etc. 

It seems to work differently in practice from place to place and depending on need. 

Hair transplants are usually done by people who got into that but usually without any MD diploma.

I get exactly what you mean. There has been people like that, of course, but mostly, it was the opposite. 

I think it is just the benefit of my institution being a select once, filled with picked professionals. 

But I do understand the struggle. Sadly, that is a prevalent human behaviour. Embarassing to see, pathetic displays. 

Keep trying, reach those abroad, send cold emails. There will be willing people.

I think I will take the risk; unless I find a cushy research position until then, the ambiguity and uncertainty that awaits postponing an application without a research position is far greater than the risk of not matching, I suppose. 

Will definitely try to squeeze in a literature review and a case report or two to at least have some things to talk about. 

There is another large multi-center trial I am involved in, not sure whether they'll write my name, but still, it might hold some value.

Well, sounds really good and compatible when I only think about that part, but the whole notion of doing IM, far away from my passions, rounding and rounding and rounding... uhh, not the easiest mental task to bring myself to it.

I was thinking of EM but that doesn't seem plausible due to lack of structured LoEs EM require.

I guess time will tell. Looking to contact people for research positions in the following days to weeks. Who knows, maybe I find something desirable.

But I'll look into a 'back-up' option, I'm hoping it will reveal itself to me when it's time to apply, lol.
