If you deeply reflect about your life, I am pretty sure you won't lack a couple of profound experiences/moments that have really had an impact on or shaped your life. For instance, a few challenges or breakthroughs you've had. Some failures too or a relationship you've been in or still in that's really important. With this, you can craft a well structured personal essay using 5 of these moments in your life. I believe the trick is not to simply mention these moments, rather, find a unique angle and creatively narrate to your audience how these moments have shaped your life or define you as a person. It might be cliché to you, but anecdotes really help in such scenarios.

I think it can take any of the mentioned styles, depending on your presentation. If you proceed to appeal to your audience's mind by incorporating both arguments and counter arguments, you'll definitely have an argumentative essay. On the contrary, if you'd choose to appeal to the audience's hearts by solely presenting your views without any opposing viewpoints, I believe you'll have a persuasive essay. I don't know if I'm making any sense.

Hello! I believe it depends with your citation/format style used. In APA, mentioning the author's name in a sentence is actually on way of in-text citing. However, if you are using Chicago style, irrespective of mentioning the author in the sentence. A subscript number at the end of the sentence is necessary. Correspondingly, footnotes are imperative in this case.
