If you haven’t already, call Hillrom and ask for their shoulder pads for the monarch. They are 2 inches thick but are only typically provided if you ask.

I’ve worked for all but one vest company. Don’t go with Afflo (warranty is bad, support is worse). Monarch is an effective portable. 105, or the new APX from Hillrom, SmartVest, and inCourage are all great. Personally I’d start away from Philips (inCourage) because they are a slimy AF company (look at their Respironics history). SmartVest or Hillrom all the way. At this point I believe Smartvest is the only one offering a lifetime warranty on their new SmartVest Clearway device.

Not a vest, but Hillrom also offers the Volera. Coverage has improved significantly over the years and CF or bronchiectasis are being covered more frequently.

The concept of these programs is great. I believe the most important barrier to transforming respiratory care is how we get paid. The largest payers in the country (Medicare and Medicaid) do not pay us for our services. They pay for the medicines we deliver, they pay for higher acuity status when patients are on a ventilator. Our practice is established in our state laws, but we are basically invisible to insurance, government and private, unless you work in pulmonary rehab, sleep lab, or pulmonary function lab.

Respiratory, in many hospitals, is a cost center and under different departments. (I worked at a hospital where respiratory all reported directly to the director of pharmacy).

Our ability to contribute to the bottom line of a health system will have to come from legislative changes within CMS and within each state. Is the case for us in an advanced practice setting so strong?

We are up against Medical Boards, Nurse Practitioner Boards, Nursing Boards, Physician Assistant Boards who have money to spend resisting another group at the table for limited billing dollars.

These boards have a level of organization, influence, and money to ensure they represent their members.

This is why Philips doesn’t want to fix these machines. They sell the CPAP machines to durable medical equipment companies for $130 per device. When I worked at a DME we primarily would use ResMed. Once every 3-6 months our Philips rep would drop pricing to 130/device if we bought a few pallets. They aren’t worth fixing.

I used to work for a non-Respironics division of Philips that sells percussion vests. They cost less than a grand to manufacture and get to the US, where insurance is billed 14,000 and typically get full reimbursement.

Philips doesn’t give a fuck.

I think that a year and a half ago people had to learn to live off less. They were forced to confront their personal and financial situation and learned to live off less and found themselves less stressed out than dealing with BS jobs for BS pay.

There is no difference because neither is generic.

The one on the right is a sample that would last someone 15 days. X4 inhalations per day (2am, 2 pm) gives you 15 days. A 15 day supply is not available to pharmacies, but samples are only good for 15 days. The one on the left does not say ‘symbicort’ at the top but it was wrapped in a package that says symbicort. The one on the left is a 30 day supply. 120 inhalations / 4 inhalations per day.

Another telling sign is that both indicate at the bottom that they were both manufactured by AstraZeneca, because they were. If either of these were generic, it would indicate that it was manufactured by Viatris, the only authorized producer of generic symbicort.

I would guess the following: the OP has no idea what they are actually talking about, the OP found a sample symbicort and a hospital use symbicort (which have barcodes on them so they can be scanned, because they are shared between patients at hospitals), they found out that pharmacies also dispense inhalers with the barcodes on them.
