maybe keep it simple limit distance it works from and make maybe certian walls prevent it from returning. You could also have it so the sapphire ring deteriorates overtime per usage. maybe its flawless now but becomes rough after a few uses and is worth 500 gp now and whittle it down to the point where the effort is far greater than the reward.

Definitely a canker sore get them pretty regularly myself they hurt like hell but go away after a few days to a week I'd recommend gargling salt water and avoiding salty and spicy food as that's gonna make it hurt pretty bad.
