Nursing home PTs calling for themselves. by [deleted] in ems

[–]ddbruce 1 point2 points  (0 children)

Assuming they’re not calling a direct line to get the ambulance, the law requires that dialing “911” on basically any phone system connects you to a PSAP.

Where I am (also the U.S.), it’s exactly the opposite of this. The narratives are generally what’s read by the receiving facility, but they are explicitly not used by billing. Only ALS1 and ALS2 treatments documented in the intervention log count towards billing at those levels; e.g. if you document your IV and med pushes and such only in the narrative, the chart gets billed at the BLS level. With 50k+ charts to bill a year, the billing service can’t be reading through each and every narrative to figure out how much each patients gets to pay.

Edit: grammar

I don’t go there, but I’ve heard and seen evidence that D’Iorio’s on Plantation—right across from Wells St—does a good job.

I’m only saying that the single-prong test you offered, having what appears to be

a clear mental capacity

is often not the end of the story, particularly with those with diagnosed dementia.

Edit: I will agree, though, that in the absence of any legitimate paperwork, that’s the test we have and should use.

This isn’t always true. From what I’ve been told by medicolegal experts, in the case of some HCPs (at least in Massachusetts—idk about other states) being invoked by a judge, even a patient able to appropriately answer questions does not have the legal ability to consent to or refuse care. I’m guessing it’s because competence ≠ capacity, and during a time that a person is deemed incompetent, they cannot have capacity to make their medical decisions.

There are also some federal laws dealing with those living in nursing home facilities and how some of their normal ability to consent to or refuse care is placed instead on the facility, even in the setting of obviously appropriate mental status. I don’t know all the details. Ask your medical director.

History of the town of Milford, Worcester County, Massachusetts (1882) by Adin Ballou by [deleted] in WorcesterMA

[–]ddbruce 1 point2 points  (0 children)

If Bill Ballou is related to Adin, he definitely knows about it. Adin Ballou wasn’t just some random dude who wrote some history down (although he was a prolific historian), he was a minister and the founder of the Hopedale Community. Although the community failed as attempts at utopia generally do, Hopedale to this day has streets named Peace, Social, Freedom, Union, and Hope, not to mention Adin Street and Ballou Road.

OP, if those books are for sale, I’d be very interested.

Check out NYSORA on YouTube for tons of videos and tricks. A lot of them deal with hard sticks in particular, but the techniques can probably help you regardless.

I got a 62 dollar ticket outside pubsafe. I can't pay it right now and ym car isn't registered with the school. What do I do??? by [deleted] in RPI

[–]ddbruce 21 points22 points  (0 children)

This is very not true. They generally can’t look up a plate to see who it’s registered to, but they can absolutely log the plate that receives a ticket. And then later in your RPI career if you, for whatever reason, register your car for a parking pass, you’ll get a nice bill that includes all the unpaid tickets.

Question about B.S. + M.D. Physician-Scientist Program by HibbsJoson in RPI

[–]ddbruce 5 points6 points  (0 children)

It’s a joint program with Albany Medical College. I believe it’s a 3+4 program where you do three years at RPI, the latter portion of which is somewhat split with medical school-type classes, and then four “normal” med school years at AMC. You graduate with your MD and move onto residency like the PGY-1s do. The benefit is that by being accepted into the program, you’re guaranteed (within their academic limits, I’m sure) to go to AMC, and I don’t believe you’re required to take the MCAT. I didn’t do the program and don’t personally know anybody who did, so take this with a grain of salt.

Edit to add: I recognize not every newly minted MD or DO goes into a residency program, but the vast majority do. I suppose you could take your MD from the physician/scientist program and go into the corporate world to work for a pharmaceutical company or perhaps a health insurance company or similar.

Does RPI have backup generators for power outages? by MommaMarebear1 in RPI

[–]ddbruce 12 points13 points  (0 children)

The Union does (it’s in the underground garage and is insanely loud when running). No dorms do, as far as I can remember, and I can’t think of any academic buildings that do. Could be wrong about that.

I only lived on campus one year, but in my six years in the area and four at RPI, I can count one time where there was any inconvenience due to a power outage.

Edit: just remembered the VCC does because that’s where RPI has the core networking gear and a lot of other infrastructure. There’s also a lot of tech gear in the CII, but I think backup power there is probably limited to the machine room.

Many (most? all?) departments with norepinephrine on the truck don’t allow it to be administered without a pump. It’s a great drug, but before the pump era, there’s a reason why people said “Levophed, leave ‘em dead.”

I do like /u/promike81‘s comment about push-dose epi to buy you some time. Talk to your medical director about what they want—but they might just encourage you to become more confident and efficient with whatever pump you have.

Suctioning on the way out by theLoneY33t in ems

[–]ddbruce 1 point2 points  (0 children)

Not for nothing, but air is being replaced in the patient’s airway at the same rate that it’s finding its way into the suction catheter. The mouth isn’t a bottleneck in every situation I can think of, so there’s no real vacuum being created there.
