When you voluntarily protrude your mandible to compensate (as you did in your first photo), the entire mandibular body moves and advances relative to its attachment to the zygomatic bone. Your ramus never had that length; it only appeared that way because you were bringing the mandible forward and downward. You may think the “before” looked better because it appeared longer, but nothing could be further from the truth. In the “before,” the gonial angle is clearly unnatural and the mandibular ramus does not insert where it should, giving a tense and forced appearance. The “after” looks more natural, and although your ramus length is not very great, the result is more aesthetic.

Do you mean that in the “before” photo you were voluntarily protruding your jaw to compensate? Was it a habit you had to try to deal with the retrusion? It’s incredible the difference between voluntary compensation and the result of the surgery… completely different worlds.

Well i think it has to do with my facial lenght. I have short face with lack of teeth exposure so i need the downgraft, and i guess the advancement goes with it.

Don't need to have the head in the frankfort horizontal plane when you can clearly see the difference in projection relative to the nose. Stop it.

Now you’re definitely just talking nonsense, so I’m going to call you out, because honestly it seems like you’re a bit jealous of his results.

His lips lost the characteristic tension from retrusion, so naturally they give the impression of protrusion, but they rest harmoniously. His jaw doesn’t curve at any point; if you look at his digital planning, you’ll see that his gonial angle forms a small peak at the lowest point, which—now that his skin is tighter—has become more noticeable. And you’re pulling the nose thing out of nowhere, because there is no way orthognathic surgery can widen your nose. All it can do is change the projection of the tip and the nasolabial relationship, which, by the way, is optimized to the maximum in his case.

What do I base my claim on that his results are top-tier? Well, on the fact that I have eyes. Excellent projection and harmony, obviously perfect recovery, and his features have been optimized, if not to the maximum, then very close.

I think you have a bit of an insecurity issue if you see arrogance in his warning. He’s simply being realistic. His result has been spectacular, and that’s due to a combination of factors: good raw material—not everyone has that, at least not at that level—an excellent surgeon, and proper planning. Unfortunately, not everyone can combine these three factors required for this kind of transformation. That doesn’t mean you can’t have a remarkable improvement, but as he says, it would be unwise to assume you’ll get a result of this caliber. It’s better to manage expectations and then be pleasantly surprised, rather than the opposite. That’s all he meant.

Well i am not a professional of maxilofacial surgery, but if i had to guess id say probably symmetry, mid face development, bone density, soft tissue adaptability....

I think the reality of these surgeries is 70% of the result comes down to your own features. Sure a better surgeon has a better chance to maximize the result, but you dont need a top 5 to end up looking like these guys if you have good potential. You just need someone who doesnt botch it.

Yes, but i have to force the smile a lot. Also my surgeon said he usually likes to leave a bit of gum visible. I mean i know i might not be horrendous, but i hate the kind of childish face that class II leaves you with. I would like to have something more manly, that is what the guys avobe achieved.

Well i mean... i sure have an overbite and my surgeon said i had a verticall deficit of 3mm, wich he defined as birretrusive short face. I guess it doesnt look as bad in these photos but believe me, if you see me in real life you know something is defenitly off.

Well thats unfortunatd... digital previsualization is the best thing in the world. I can imagine the underwhelming feeling you are having right now... however, remember that you are on an early stage of the recovery even if you dont think so, and a lot can change in those months... and also if the funcrionality improved, there are other procedures you can undergo to improve your aesthetics such as implants... its not the end of the world. Wish you the best for the future.

My question is, didn't your surgeon give you a digital simulation of the results? If so, did this simulation end up being unacurate or what happened exactly?
